Defining Social Problems
In this course, Design and Evaluation of Programs and Projects, you examine aspects of program planning and evaluation while also engaging in a Practicum Experience. As you do this, you may notice the differing terminologies and approaches that are applied in various circumstances. For instance, you are likely quite familiar with the phrase “health problem” from your previous coursework and professional practice. This Discussion looks at understanding “social problems” as part of a framework for program design. What is the distinction between these terms? Why is it important to notice this divergence?
Problem analysis is a cornerstone for effective program planning and should be conducted at the outset. With this first Discussion, begin to pay close attention to the language and perspectives that will inform your program planning work as you move forward in the course.
 

To prepare:

  • Consider      the following scenario:
    • Data from       the Appalachian region show lower numbers of women receiving mammograms       compared to the national average, indicating a need to increase use of       this procedure in this area. However, the data also show that women from       this region are reluctant to participate because of their attitudes       toward mammograms (Royse & Dignan, 2009).
  • Review      Chapter 1 of Designing and Managing Programs to be sure      you have a clear understanding of the sequencing of program design and      evaluation, as well as the importance of each element of this process.
  • Then,      review Chapter 3. Analyze the scenario above in light of the concepts      presented:
    • Why is it       important to avoid stating the problem as a solution?
    • How does       this scenario illustrate a “social problem”?
    • What       responses to the problem analysis framework questions (pp. 45–49) could       you develop given the information provided?

By tomorrow Tuesday 11/27/18 at 2 pm, write a minimum of 550 words essays in APA format with at least 3 references from the list of REQUIRED READINGS below. Include the level one headers as numbered below:

Post a cohesive scholarly response that addresses the following:

1) Describe three key insights or strategies you would share with your team if you were engaged in planning for a program related to the scenario described above.

2) How do you expect that your intended approach to developing an understanding of social problems could affect program planning? Be sure to support your response.

Chapter 3 emphasizes critical considerations for developing an understanding of a social problem that may differ from how this has been approached by service providers in the past-including the importance of beginning with problem analysis early in the planning process and focusing on understanding the problem rather than generating solutions.

Researchers use instruments as a measurement device and they can be categorized as researcher-completed and subject completed depending on who administered or completed respectively. Based on the research question, researchers select what instrument they are going to use.

Some researcher-completed instruments are rating scales, interviews, flowcharts, checklists, time and-motion logs, observation forms, etc. Some of the subject-completed instruments are questionnaires, attitude scales, personality inventories, achievement tests, sociometric devices, etc.

In my proposed research project I am going to use a the Readmission Rate , based on a relation between the total number of patients discharged home with the diagnosis of Congestive Heart Failure and an inpatient admission for decompensating Cardiac Heart Failure within 30 days of the discharge .This number is going to be the numerator of the fraction. The denominator is going to be those all patients discharge from the hospital with the diagnosis of Cardiac Heart Failure and the number is going in the last three months. The denominator is going to be common for the two groups the experimental group and the control group.

All patients included in the investigation will be applied the LACE Index. The LACE Index is a tool to evaluate the Risk of readmission of a patient admitted in the hospital, and it is used as a predictor of readmissions. It consists of 4 parameters as described below:

Length of stay It will score 0,1,2,3,4,5,7 according to the days of stay: less than 1, 1, 2,3,4-6, 7-13, >=14 respectively.

Acuity if it was emergent admission it will score 3.

Comorbidities will be assessed by Charlson score .No comorbidities will score 0, 1 Comorbidity scores 1, 2 Comorbidities scores 2, 3 comorbidities scores 3, and more than 4 scores 5.

Emergency department visits on the last six months No visits 0, 1 visit scores 1, 2 visits scores 2, 3 visits scores 3, and 4 or more scores 4.According to the classification of the measurement tools this level of measurement is considered as ordinal.

The interpretation of the LACE Index is that if the Index scores is 0-3 there is a low risk of readmissions, if it is between 4 and 9 the risk is moderate , and if it is more than 9 the risk is high.

This past spring, renowned nurse researcher Linda Aiken co-authored a study that contributes to a growing body of evidence suggesting that a more educated nursing workforce translates into better patient outcomes. “Among the conclusions made by Aiken was that patients in hospitals in which 60% of nurses had bachelor’s degrees and nurses cared for an average of six patients would have almost 30% lower mortality than patients in hospitals in which only 30% of nurses had bachelor’s degrees and nurses cared for an average of eight patients.

2-From my experience all nurses including BSN, Diploma and Associate Degree RN’s have better understanding of the entire healthcare system and an in-depth understanding about a patient’s overall history that automatically helps them make better and faster decisions, make fewer errors and better guide the patients and their families. However times are changing and rapidly expanding clinical knowledge and mounting complexities in health care mandate that professional nurses possess educational preparation commensurate with the diversified responsibilities required of them. As health care shifts from hospital-centered, inpatient care to more primary and preventive care throughout the community, the health system requires registered nurses who not only can practice across multiple settings – both within and beyond hospitals – but can function with more independence in clinical decision making, case management, provision of direct bedside care, supervision of unlicensed aides and other support personnel, guiding patients through the maze of health care resources, and educating patients on treatment regimens and adoption of healthy lifestyles. Having a BSN degree allows more opportunity for employment, increased responsibility, and career progression.

3-The capacity of a nurse to deliver quality care and safeguard the safety of a patient is dependent on the nature of training in nursing school. Anbari and Vogelsmeier (2018) explored the perceived benefits of Associate Degree in Nursing (ADN) and Bachelor of Science in Nursing (BSN) on the capacity of nurses to uphold patient safety in the course of service delivery. The duo engaged ADN-to-BSN graduates to find out the perceived implications on education on their capacity to promote the safety of the patients. Attainment of BSN qualifications expanded the nurses’ clinical reasoning, as they can approach care with a broadened scope as well as accept inputs from other people. Through the paradigm shift in the delivery of care, it can be argued that advanced training of nurses is instrumental in the enhancement of making decisions that conform to the needs of patients. In turn, this predisposes improved patient safety. However, some nurses believed that BSN is essential for career progression rather than improved their capacity to uphold patient safety.

From a personal viewpoint and based on my experiences, I consider BSN critical for the improvement of patient safety. In the course of acquiring the qualifications, nurses learn about new concepts in nursing and are exposed to approaches that may be helpful in the management of complex situations that may arise in the clinical setting. Through the learned concepts, nurses can significantly improve their clinical reasoning, as well as engage other healthcare professions. Through this, they are likely to apply evidence-based practice and limit engagement in behaviors that may adversely affect the safety of patients.
