Patient Centered Care

In patient centered care, nurses and other medical professionals take care of patients and their families in valuable and meaningful ways to get the best results and hence characterized by several principles. The first characteristic of patient centered care is the exploration of the disease and illness experience (Masters, 2018). By exploring how the patient and his family feels about the disease or illness in question, it is possible to develop appropriate measures for dealing with it. Believes, perceptions and attitudes about a disease are vital determinants of how a patient responds to treatment against a disease.

The second characteristic of patient centered care is that it is understanding the whole person. A nurse ought to understand a patient including their physical and emotional attributes that affect treatment (Masters, 2018). This allow him to respond to their needs in appropriate methods. The third characteristic is that patient centered care in culturally appropriate. The culture of the patient should be put into consideration in developing the care process. By respecting diversity, patient centered care ensure that healthcare is delivered in a manner that the patient understand hence increasing chances of recuperation.

Patient centered care should also be based on teamwork and collaboration. There is a need for development of measures to promote collaboration among healthcare staff working with patients (Masters, 2018). Collaboration and teamwork eliminate errors, and presents the best solutions in patient care.

Patient Education

The first step to providing patient education is understanding their cultural backgrounds. Patient centered-care include culturally appropriate education (APA., 2009). Educations should be done in the language they most understand with minimum use of medical jargon. Secondly, the nurse should understand the patient’s family background. Members of the family should be included during patient education as they provide support during the recuperation process.

The third consideration to make is in regard to what the patient and his family members already know about management of the disease. Including members of the family in patient education increase the chances that it will be followed (Haley et al., 2017). While it is not necessary to repeat what the patient already knows, confirmations should be done to ensure their knowledge is accurate.

The use of visual aids is also necessary during the education of this patient. Charts, pictures, PowerPoint and even videos if possible should be included in the education. The nurse should take advantage of technology and provide videos and other material to reinforce the teaching (APA., 2009). Moreover, the nurse can demonstrate certain procedures using available items and other people available.

Evaluating the Effectiveness of Patient Education

To evaluate the effectiveness of the education in the scenario, several methods can be used. First, the nurse should ask the patient to restate the instructions given (Haley et al., 2017). For example, the patient can be asked to restate the drug regime in their own words. The patients should also be presented with questions to help identify areas where reinforcement teaching may be needed.

The nurse should also evaluate the patient and his family for attitude towards the teachings provided. To evaluate for attitude, the nurse should ask the patient and his family how they are going to incorporate the information in their daily routines (Haley et al., 2017). For example, one can ask the patient how they will incorporate their drug taking regime in their daily schedules.

The last method of evaluation will involve looking at the outcomes of the education provided. During clinical visits, the nurse will ask whether the patient has been able to follow the education provided (APA., 2009). Also, the outcomes of treatment including the effectiveness of management of the condition will be examined.

Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.

Your spiritual needs assessment survey must include a minimum of five questions that can be answered during the interview. During the interview, document the interviewee’s responses.

The transcript should include the questions asked and the answers provided. Be sure to record the responses during the interview by taking detailed notes. Omit specific names and other personal information through which the interviewee can be determined.

Part II: Analysis

Write a 500-750 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead, provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:

1. What went well?
2. Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
3. How can this tool assist you in providing appropriate interventions to meet the needs of your patient?
4. Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.

While nurses have high turn-over, it is higher with home health aides. During the research for this assignment, the information I was able to find in regards to HHA and home care setting turnover, information is few and far between. The possible impact that this shortage can have on nursing from my perspective: high turnover is more likely to breed less experienced staff with many fleeing the profession. Shortage of home health aide can affect nursing through home health care agencies lacking the ability to provide services for elderly at home “a lack of skilled aides forces individuals to rely more on family, neighbors and friends, and increases the chance that they will end up in the Emergency Room” (Wexler, 2017).

It also increases the likelihood of frequent hospital admission. Having more admissions adds pressure to the ever mounting healthcare cost for the country, and impacts jobs. Services provided by nurse practitioners in the home care setting would be affected with less people at home living in the community. Homecare is still plagued with the same clinical or staff issues that an acute care setting might come across. and when those type of issues take place, then patients’ care suffer, for inpatient and outpatient activities alike.

The issue that the organization is currently facing is an increase in hospital acquired pressure injuries (HAPI’s). The hospital started noticing an increase trend in HAPIs in patients, which is a big matter of concern. Currently there is a wound care team that does monthly surveys on wounds and prevalence trends. The organization started several initiatives and investigations to see what is going on and possible solutions. During the investigation’s, management found gaps in turning for high risk patients, gaps in documenting on Braden risk assessment, and gaps in care documentation. In this some causes may be that nurses and nurse aides feel overwhelmed with the amount and high acuity of patients they have to care for. I personally saw where the nurses and nurses’ aides looked very busy and overwhelmed with all the tasks they needed to complete.

One implication of nursing is for them to involve clinical leadership in a way that leadership can see what the staff goes through and why there are gaps in turning and documentation. One journal article stated that by using positive clinical leadership to support a culture of pressure injury prevention. In the article it states that pressure injury prevention. In the article it states that pressure injury rounds were done by leadership and they looked at trends on equipment being used, how the ward feels about their trend, what the ward would like more help with, and look around the ward including review of the equipment and clinical assessments. The results were that the rounds led to safety huddles focused on skin topics (Sage & Tudor, 2017)>
