The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by the Commission on Collegiate Nursing Education (CCNE) and the American Association of Colleges of Nursing (AACN), using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, specific care discipline, and local communities.

This assignment consists of both an interview and a PowerPoint (PPT) presentation.

Assessment/Interview

Select a community of interest in your region. Perform a physical assessment of the community.

1. Perform a direct assessment of a community of interest using the “Functional Health Patterns Community Assessment Guide.”
2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Interview Guidelines

Interviews can take place in-person, by phone, or by Skype.

Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.

Complete the “Provider Interview Acknowledgement Form” prior to conducting the interview. Submit this document separately in its respective drop box.

Compile key findings from the interview, including the interview questions used, and submit these with the presentation.

PowerPoint Presentation

Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.

Include the following in your presentation:

1. Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
2. Summary of community assessment: (a) funding sources and (b) partnerships.
3. Summary of interview with community health/public health provider.
4. Identification of an issue that is lacking or an opportunity for health promotion.
5. A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.

Diabetes mellitus is characterized by glucose intolerance, chronic hyperglycemia, and disturbances of carbohydrate, protein, and fat metabolism (Huether & Mccance, 2016). Generally, a diagnosis of diabetes mellitus is based on elevated plasma glucose concentrations and measurement of glycosylated hemoglobin (A1C). Genetic susceptibility in combination with other environmental risk factors such as obesity place someone at higher risk for DM II. Obesity contributes to metabolic syndrome, altered adipokines, increased fatty acids, inflammation, and hyperinsulinemia, increasing insulin resistance further (Huether & Mccance, 2016).

Acute complications such as diabetic ketoacidosis and hyperosmolar hyperglycemic nonketotic syndrome are life threatening (in the short term), but not as common as the long-term life-threatening chronic complications of diabetes mellitus. Most long-term complications related to hyperglycemia are sequelae of hyperglycemia on the micro and macro vasculature.

According to (Lewis, Bucher, Heitkemper, & Harding, 2017), angiopathy is the leading cause of end organ disease secondary to chronic hyperglycemia. There are several theories as to why these complications exist that include toxic byproducts of high glucose metabolism such as sorbitol, the collection of glucose molecules in the basement membranes of capillaries, and a derangement of red blood cell function (Lewis, Bucher, Heitkemper, & Harding, 2017).  The microvascular complications are related to the thickening of the capillary membranes that result in micro occlusions. The micro occlusions lead to small herniations with swelling that lead to ischemia and tissue damage most commonly to tissues like the retina and the glomeruli of the kidneys (Lewis, Bucher, Heitkemper, & Harding, 2017).

the first step of the EBP process is to develop a question from the nursing practice problem of interest.

Select a practice problem of interest to use as the focus of your research.

Start with the patient and identify the clinical problems or issues that arise from clinical care.

Following the PICOT format, write a PICOT statement in your selected practice problem area of interest, which is applicable to your proposed capstone project.

Conduct a literature search to locate research articles focused on your selected practice problem of interest. This literature search should include both quantitative and qualitative peer-reviewed research articles to support your practice problem.

Select six peer-reviewed research articles which will be utilized through the next 5 weeks as reference sources. Be sure that some of the articles use qualitative research and that some use quantitative research. Create a reference list in which the six articles are listed. Beneath each reference include the article’s abstract. The completed assignment should have a title page and a reference list with abstracts.

Suggestions for locating qualitative and quantitative research articles from credible sources:

1. Use a library database such as CINAHL Complete for your search.
2. Using the advanced search page check the box beside “Research Article” in the “Limit Your Results” section.
3. When setting up the search you can type your topic in the top box, then add quantitative or qualitative as a search term in one of the lower boxes. Research articles often are described as qualitative or quantitative.

To determine what research design was used, review the abstract and the methods section of the article. The author will provide a description of data collection using qualitative or quantitative methods.
