Scenario – Jessica, a registered nurse (RN), and her husband are finally leaving on their vacation trip. They pull up at a red light as it is about to change to green. They watch in horror as a large truck, moving fast down a hill, in unable to stop before crashing into a van carrying a mother and her child. The van is thrown into the air and lands in a small park. Jessica runs over to offer aid. She finds a semi-conscious woman in the driver’s seat and an unconscious 4-year-old little boy in the back seat strapped into his car seat. The mother asks Jessica if Christopher is all right before she slips into unconsciousness. Christopher is unconscious and not breathing with his head down and touching his chest. He has a gash bleeding on the side of his head caused by his tricycle flying over the back seat during the crash. A truck driver, who also stopped to give help, yells in the window at Jessica, “Don’t move him!” Jessica knows that she has to get Christopher’s breathing started. Even though the truck driver is still yelling at her not to move the boy, she gently lifts his head off his chest. Christopher, still unconscious, starts breathing immediately. Jessica stops the bleeding on his head by applying pressure using a clean handkerchief from her husband. Emergency help arrives ten minutes later.

Like a firework spreading out over the night sky, the history of accreditation overarches much of health care’s past. In the late 1800s, the emphasis on health care practices, procedures, and expectations was much like a fuse that propels a firework into the air. Uniformity of high quality standards was the new goal to reach. As this new goal ignited however, many different organizations and committees began to form, each branching out in its own direction. One association, the American Society of Superintendents of Training Schools of Nursing, pushed to equalize and elevate standards across all schools of nursing. Today, this association is known as the National League for Nursing (NLN), a specialized accrediting body for nursing programs. In 1913, another association, the American College of Surgeons (ACS), began to conduct on-site inspections of hospitals. Out of the 692 hospitals they visited, only 89 met their minimum quality standards. Like the NLN, the ACS continued to grow, later merging with what is known today as the Joint Commission.

In this Discussion, you explore the many accrediting bodies and regulatory agencies that accredit and certify academic and clinical settings.

To prepare:

  • Review this week’s Learning Resources on accrediting bodies and the overarching accreditation process for academic settings.
  • Review Chapter 2, “Curriculum Development and Approval Processes in Changing Educational Environments,”  in the Keating text. Consider how regulatory agencies accredit and certify clinical settings. In addition, consider why a clinical setting might seek accreditation or certification.
  • Consider the setting your team is investigating for your Course Project. Mila Jefferies is a recently widowed 36-year-old mother of two children and the daughter of two aging parents in the southeastern United States. She and her children have recently relocated from an urban neighborhood to a rural town to care for her parents, Robert and Susan. The move involved a job change for Elizabeth, a change in schools for the children, and an increased distance from the family of the children’s deceased father. Mila’s older child is a 5-year-old daughter, recently diagnosed with autism spectrum disorder and dyslexia. The younger of the two children is a 3-year-old boy with asthma that has been difficult to control since the move. Robert is a 72-year-old Methodist minister who recently suffered a stroke, leaving him with diminished motor function on his left side and difficulty swallowing. Susan is 68 years old and suffers from fibromyalgia, limiting her ability to assist with the daily care of her husband. She has experienced an increase in generalized pain, difficulty sleeping, and worsening fatigue since her husband’s stroke.
