According to Burns, Gray and Grove (2015) “Qualitative research is a systematic approach used to describe experiences and situations from the perspective of the person in the situation. 2-Qualitative data can be difficult to process and analyze if not organized properly but the organizing itself is often a difficult task because qualitative data by its very meaning has no numerical data that can be placed in any order or sorted. For this very reason, the data almost always has to be separated and sorted by a human being rather than a computer system that would have to be programed to search for a numerical characteristic that doesn’t exist. In the place of these numerical characteristics, the researcher would need to sort their data into themes based around patterns found in their work. These patterns would differ based on the nature of their research. Once these patterns were decided upon they would be used to create groupings for the data.

3-“Qualitative research is multimethod in focus, involving an interpretive, naturalistic approach to its subject matter. This means that qualitative researchers study things in their natural settings, attempting to make sense of, or interpret, phenomena in terms of the meanings people bring to them” (Mcleod, 2008). What this means is that the point of qualitative research is to understand the reality of the subject matter and participants that are being studied, which is why these test subjects are studied in their natural setting. I agree that qualitative data can be voluminous and overwhelming, so organizing large amounts of data in to smaller groups would be preferable. One way you could start would be to divide up the data into the different environments in which they are being studied. For example, if you are assessing fall risks in the elderly in the healthcare setting, there are three different categories you can divide into in themselves. To further break down the data, you could organize any of these groups into different facilities, such as assisted living facilities, hospital, nursing homes, rehabilitation facilities, and adult foster homes. The division of data is all dependent on what the focus of your study is and what end goal you are attempting to achieve. If you are seeking safe staffing ratios, you  could begin by dividing fall risk patients by facilities with higher staffing ratios than those with lower ones to compare and contrast. There really is no “exact answer” on how to organize the data. The development of technology is very helpful in the organization of data in recent years. I believe that professor Mcdonald made a comment earlier about a study that she was once involved in that did not use computers for the organization of data. It would definitely speed up the process of sifting through the information you have for the information that can be used. When it comes to organization, in any capacity, I find that lists and spreadsheets help me to focus my mind and not leave out any important information. This method may not work for everyone, but if I don’t write something down, chances are, it will get forgotten in the end.

My PICO(T) question is; In preschool-age children (P), how does receiving the flu vaccine (I) compare to not receiving the vaccine (C) impact the risk of severe illness or death related to flu (O) during the flu season (T)? 

The primary databases I used were Pubmed and CINAHL Plus with full text.  The search terms I used were; Flu Vaccine; and Children; Effectiveness of Flu Vaccine and Children; Flu Vaccine and Children and School Age; Effectiveness of Flu Vaccine and Statistics and Children.  

When I searched CINAHL, there were 1,798 results.  I narrowed the search to peer-reviewed articles and changed the dates, so they ranged from 2015 to 2020 to show the most recent articles in evidence-based practice.  After adding these Boolean operators, I had 176 peer-reviewed articles.  Walden Library (2020), recommends using Boolean terms to create a more precise search with more relevant results.  A recent study concluded that it is essential to search at least three different databases to retrieve approximately 90% of all the relevant literature on a subject (Ho, Liew, Ng, Shunmugam, & Glasziou, 2016). 

A strategy to increase the rigor and effectiveness of a database search is to use alternative terms.  I could try searching alternative words such as “Influenza,” “juvenile,” or “youth” to improve my results.  On PubMed, I used the advanced search filters to narrow my search.  I clicked to view articles that only studied humans and that were published within the past five years.  Another strategy is to utilize a feature on PubMed called Clinical Inquiries.  Clinical Inquiry is designed to improve search results by linking the type of question, therapy, diagnosis, or prognosis to a stored search strategy that retrieves the appropriate research methodology (Schardt, 2007).  Utilizing all of these techniques will help improve my search results to find the most relevant peer-reviewed articles for my topic.

Seth is very athletic and especially enjoys surfing. He has been surfing since the age of twelve and he started making surfboards while he was in high school. After obtaining a college degree in marketing Seth decided to start his own business making surfboards and giving surfing lessons for a local resort near his home. During a routine physical examination the physician assistant that Seth sees commented on the presence and appearance of a small growth (approximately 2-cm in diameter) above Seth’s right eyebrow. This growth was shiny with a pearly border, a central ulcer and engorged surface blood vessels. The physician assistant was concerned that this might be a tumor and asked Seth a number of questions to aid in the diagnosis. Seth reported that he noticed this growth approximately six months ago but thought nothing about it since he often developed “pimples” when he was working on surfboards because of the heat and dust. Seth noticed that the growth would occasionally bleed, but he thought the bleeding most often occurred when he absently scratched above his eyebrows when he was working. Seth also reported that after bleeding the growth would “crust over” and appear to heal, but not disappear. Seth also thinks the lesion has increased in size, but he can’t be certain.

The physician assistant asks Seth to come back in two days to visit with the attending physician and encourages Seth to be sure to use sunscreen whenever possible, especially when he is surfing or teaching surfing classes. Upon returning to see the physician, Seth is told that he probably has a basal cell carcinoma. This, the physician explains, is a cancer that while not benign, rarely metastasizes and grows very slowly. This type of cancer usually occurs in areas that have the greatest exposure to the sun (i.e. the face, head, and neck). While this cancer would probably not metastasize, the physician suggests surgical removal to minimize damage to any underlying tissues and a histological examination of the excised tissue to confirm the diagnosis.

Seth is concerned that if this is indeed cancer he will have to undergo radiation and/or chemotherapy and suffer from both hair loss and nausea like his mother did when she was being treated for breast cancer 10 years ago. The physician assures Seth that these treatments are not required for basal cell carcinoma and explains that this cancer usually remains localized to the specific area of the skin and does not usually spread to other tissues, so radiation and chemotherapy would not be necessary. Seth then asks why it is necessary to remove the tumor if it will not spread and is told that while the tumor does not spread to other body areas it will continue to grow and also invade deeper layers of the skin, such as the dermis, and destroy normal tissues. Seth agrees to the procedure and schedules a follow-up appointment to discuss the results of the biopsy the following week. At the follow-up appointment, Seth is told that the lesion was indeed a basal cell carcinoma. He is told that it will be important for him to try to use both sunscreens, specifically those containing para-aminobenzoic acid or PABA, and protective clothing as much as possible to minimize the effects of the ultraviolet (UV) rays of the sunlight. Seth is also told that while he should not be overly concerned, he should also pay close attention for similar lesions in the future, since a person with a single basal cell carcinoma is likely to develop additional basal cell carcinomas in the future.

I feel that the most important objective for the older adults are to decrease the rate of emergency department visit due to falls. As adults increase in age they become more at risk for falls which can lower their quality of life or lead to death. Falls usually occur for one of these reasons chronic illnesses, impaired vision, environmental hazards and medications. “Every year, 3 million older people are treated in the emergency department for fall injuries” (CDC, 2017).  We can prevent falls by increasing awareness of falls and educating patients and their families about ways to prevent falls. There are ways to prevent falls such as having your doctor review your medication, have an eye exam once a year and participating in activity for strength and balance. Also make your home safer by making sure your home has adequate lighting and getting rid of things that will cause falls.

Health care systems in various countries have different accountabilities. One cannot just bring change by implementing any Evidence-Based Research Project directly without having understood the implications of the health care system. It is imperative to carry thorough studies to gain knowledge about the extent to which health care systems have developed and what level of changes and advancements that are needed in this regard. For proper implementation of evidence-based practice, it is essential to examine care related to individuals and how the local staff is performing from monetary and technological resources provided to the organization (Stokke, 2017). The flow of information at the hospital also has to be analyzed depending upon which are a more considerable part of the practical implementation. The medical caretakers have the heavy responsibility of implementing and devising strategies to eliminate healthcare-associated risks, answer the clinical inquiries and work on them if they are needed for the betterment of the healthcare system. The two most essential change theories, in my opinion, are Lewin’s model and Social Learning theory. Lewin’s model has remained very useful in explaining the role of power in advancement or no advancement and implication of change. Change can only take place if the joined quality of one constraint becomes noteworthy than the consolidated quality of the restricting arrangement of powers. The social cognitive theory which was initially known as social learning theory considers that the change in behavior has been primarily impacted by individual variables and some properties of behavior. In comparison to both models, Lewin is more valid and reasonable. It disregards all components related to individuals that affect change. In contrast to this, the social cognitive theory is more influenced by what flows are drawn naturally and focuses on individual components. Lewin model is also more preferred because it takes into consideration, the outer and inner ecological conditions (Moses, 2015).

The two most commonly recognized change theories are Lewin’s and  Lippitt’s change models. The two are both very similar to one another in that they both look to evaluate when change in needed, initiated, and ultimately evaluated. The differences with each  are  how the creator ultimately expands further with these three stages.  

Lewin’s Model consists of three stages: unfreezing, moving, and refreezing. The unfreezing stage ultimately looks at status quo, and increase driving forces for change; moving stage is the action stage in which the changes are implemented and involve people; and the final stage refreezing establishes the change as the new way of doing things with the reward of desired outcomes (Mitchell, 2013). Though Lewin’s model is simple and straight forward, as we all know now, change does not just happen as simple,  st raight  to the point stages. This is where Lippitt’s change model may work better not only for changes but changes to be done within the nursing field.  

Lippitt’s model directly reflects change in a way  nurses  already know how. This is true because the language used to establish the model mimics that of the nursing process (Mitchell, 2013). Lippitt’s model is broken down into 7 phases, as follows: diagnose the problem (phase 1), a ssess motivation and capacity for  change ( phase 2), assess change agent’s motivation and resources (phase 3), select progressive change objective (phase 4), choose appropriate role of the change agent (phase 5), maintain change (phase 6), and terminate the helping relationship (phase 7) (Mitchell, 2013). Use of Lippitt’s model though it may not be as direct can help create a  clearer  and outlined  way of implementing change within the health care system. This is primarily why I would more likely use Lippitt’s model because  it  acknowledges more specific areas where change can either be a halted because oversight.

Each year, 1.5 million Americans are diagnosed with diabetes (American Diabetes Association, 2019). If left untreated, diabetic patients are at risk for several alterations, including heart disease, stroke, kidney failure, neuropathy, and blindness. There are various methods for treating diabetes, many of which include some form of drug therapy. The type of diabetes as well as the patient’s behavior factors will impact treatment recommendations.

For this Discussion, you compare types of diabetes, including drug treatments for type 1, type 2, gestational, and juvenile diabetes.
