Purpose

The purpose of this PowerPoint presentation is to locate and compare performance measurement data on common health conditions for the hospitals in your area. You will investigate the Centers for Medicare and Medicaid Services websites and locate hospital compare data for hospitals within a 50-mile radius of the community where you are working or had your prelicensure clinical experiences. You will prepare a PowerPoint presentation and share the results of your findings. Opportunities for improving performance measurement indicators will be shared.

Search for hospitals within a 50-mile radius of the community where you are working or had your prelicensure clinical experiences. Type in your zip code. Select your hospital and two others. Select hospitals/facilities and choose compare. If you live in a remote area and there are no hospitals listed within a 50-mile radius, select a zip code for a family member or a close friend who does not live near you. The idea is to review comparative data.

Challenges can seem ever present in the health care field. Problems related to the quality of patient care, financial matters, technology, and interdisciplinary collaboration, for example, are often evident in organizations and professional groups.

 

In your role as a nurse leader-manager, it is important to consider challenges and how these translate into unmet needs. You can then create valuable change by addressing these unmet needs through strategic planning.

 

Through this Discussion, you may distinguish the focus for your Course Project. Your instructor and colleagues will provide feedback to help you refine your idea as you move forward.

 

To prepare:

 

Consider the information presented by Dr. Carol Huston in this week’s media program. Think about the process of moving from the identification of an unmet need through the development, implementation, and evaluation of a strategic plan.

Reflect on the initial thinking about trends and unmet needs in nursing and health care that you addressed in Week 5.

Consider how the information in this week’s Learning Resources deepens and expands your understanding of these trends and unmet needs in relation to the strategic planning process.

Proceed with planning for your Course Project as follows:

Select an unmet need that affects a specific group, unit, or organization. 

Deepen your understanding of this group, unit, or organization by examining the stated mission, vision, and values, if possible.

Analyze historical data related to the unmet need, and review the evidence in the literature. Use this information to engage in forecasting.

Consider which stakeholders should be included in the strategic planning process.

Begin to formulate a vision for addressing this need at the organizational or systems level. (Note: Although not required for this Discussion, you will need to develop a strategic goal for your Course Project.)

 

Post an explanation of the unmet need that affects a specific group, unit, or organization that you would like to focus on for your Course Project. Discussion Question

For this discussion, you are asked to review the student learning outcomes for this course and discuss how you met these outcomes. The intention here is to provide you an opportunity to “reflect” on the learning achieved during this term and how you successfully met these learning outcomes. If you do not feel you successfully met the objective, discuss your plans to successfully complete the objective in the future.

Upon completion of this course, the student should be able to:

  1. Critically analyze current practice to formulate researchable problems.
  2. Evaluate research as the basis for decision-making to improve outcomes through translation into evidence-based practice. 
  3. Synthesize an understanding of the research process through development of a proposal to address a nursing problem or focus area identified in nursing practice.
  4. Investigate ethical issues related to the conduct of scientific research, including informed consent, data management, data analysis, and protection of human subjects.
  5. Explore national and international initiatives and research priorities.

 

I feel that I have gained a core of formulating research questions to problems. I feel that I have gained a broad perspective improving outcomes with evidence based practice. The proposal development was a challenge initially, but everything seemed to fall into place until the statistical portion of the class. At that point, that is where I struggled and I feel that it is because I am not mathematically inclined.
Even though I knew about the Tuskegee incident, I learned more about the incident. This new gained information was an eye-opener for me, and I was more horrified at the events. I learned some new ethical issue related information regarding the informed consents and how difficult it was to write in “layman’s” terms as well as more information on the protection of human subjects. Most of the ethical information to me was understood or common knowledge and made sense.
Research is a process of discovering new knowledge. The charter rules (45 CFR 46.102 (d)) have been defined as “related to the survey results.” The purpose of the National Academy of Sciences is to “broaden the knowledge of the people in the physical, biological and social world” as well as the study of the knowledge of other types of knowledge reading, etc.), using a systematic process called a different scientific approach.
As far as the exploration of national and international initiatives and research priorities, this information is minimal. I feel that I have a lot to learn and that this class was only an exposure. Also, I feel that I need to explore these topics beyond this class to further myself and the core knowledge that was gained from this course.
Although I have learned to research, I feel that I am no expert but have gained good base knowledge. I am supposed to be able to form my own research. However, I feel that again with a good basic familiarity; I will probably never do my own research. Instead, I would be involved with research done by others or create performance improvements. I do not feel that I can create my own research trial all by myself and therefore would learn by others. This was an opportunity to “get my feet wet.” Additionally, since I am somewhat statistically challenged, I would need help with creating my own research. Therefore, I feel that I would never be alone and would bring in the people who would help me create a well-designed and well written study. In the end, it is all about teamwork.
When researches are done in the real world and money is spent and political people or doctors or scientists do the research. The research’s unique features are one of the common criteria published in academic journals. (Callaham, Michael et al., 2002) Diploma students usually need to conduct their first research as part of a dissertation.

As you establish your goals and objectives for this course, you are committing to an organized plan that will frame your practicum experience in a clinical setting, including planned activities, assessment, and achievement of defined outcomes. In particular, they must address the categories of clinical reasoning, quality in your clinical specialty, and interpersonal collaborative practice. 

For this Assignment, you will consider the areas you aim to focus on to gain practical experience as an advanced practice nurse. Then, you will develop a Practicum Experience Plan (PEP) containing the objectives you will fulfill in order to achieve your aims. In this practicum experience, when developing your goals and objectives, be sure to keep psychotherapy approaches and techniques in mind.

  • Review your Clinical Skills Self-Assessment Form you submitted last week and think about areas for which you would like to gain application-level experience and/or continued growth as an advanced practice nurse. How can your experiences in the Practicum help you achieve these aims?  
  • Review the information related to developing objectives provided in this week’s Learning Resources. Your practicum learning objectives that you want to achieve during your Practicum experience must be: 
    • Specific  
    • Measurable  
    • Attainable  
    • Results-focused  
    • Time-bound 
    • Reflective of the higher-order domains of Bloom’s taxonomy (i.e., application level and above)  
  • Discuss your professional aims and your proposed practicum objectives with your Preceptor to ascertain if the necessary resources are available at your practicum site.  
  • Select one nursing theory and one counseling theory to best guide your practice in psychotherapy. Explain why you selected these theories. Support your approach with evidence-based literature.
  • Create a timeline of practicum activities that demonstrates how you plan to meet these goals and objectives based on your practicum requirements.

4-The Collaborative Patient Care Management Model, which is a multi-disciplinary and a population-based case management model, was first introduced in the year 2006. The prime objective of the model was to target such patients, which could be classified as high-cost and high-risk. The nurses not only manage these patients but also play a vital in the development of inter-disciplinary care plan (Kimball, Joynt, Cherner, & O’Neil, 2007). The model allows to develop a highly potent care plan, which has 1) greater potential to produce the desired results, and 2) the ability to reduce the overall cost of the care to be provided without compromising on its quality.

5-Thank you for sharing your post.  I agree that the ACO’s are beneficial in helping to streamline healthcare.  I have enjoyed the creation of the EMR as it allows other doctors within the same health organization access to my record so they know what it going on with me and I do not have to remember all the details.  This can be important especially in the older population as many forget to write things down.  Providing coordinated care helps to ensure that patients are receiving the right care.  Continuity of care is important to patients as well because they do not want to repeat everything that is going on with them when it has already been asked.  I find that many patients want the same nurse back if they are going to be there for several shifts in a row as they can build a short term relationship with them.

 

 

6-Accountable Care Organizations (ACO) is a network of doctors, hospitals, care providers home care and long-term care agencies, physician group practices, and other health care entities, such as medical homes, that come together to form a network to coordinate Medicare patient care. They share financial and medical responsibility for providing coordinated care to patients in hopes of limiting unnecessary spending. ACO are beneficial to both providers and patients. Providers become eligible for bonuses if they keep their patient’s healthy, while also meeting quality targets keep a portion of the savings. Patient’s benefit from having their care coordinated instead of getting each part of their health care separately. This reduces hospital stays, emergency room visits and expensive specialist and testing services that may be repeated by multiple healthcare providers. To protect patients ACOs must meet a long list of quality measures to ensure they are not saving money by stinting on necessary care. And if a patient is unsatisfied with there ACO they can see doctors of their choice outside the network without paying more and decline to have their information shared with the ACO (KaiserHealthnews.2015).

We will focus on our outcome’s management. “The principles supporting outcomes management (OM) ascribed by Ellwood included emphasizing practice standards that providers can use to select interventions, measuring patient functional status, well-being, and disease-specific clinical outcomes, pooling outcome data on a massive scale, analyzing and disseminating outcomes, in relation to the interventions used, to appropriate decision makers and stakeholders” (LoBiondo-Wood, Haber, 2014, pg. 225)

Expected Change Outcomes

Our expected outcome would be patients with COPD who practice relaxation techniques versus not using relaxation techniques would see an improvement in their dyspnea and anxiety in a years’ time. Our goal would to be better control our patient’s COPD and decrease exacerbations and dyspnea. Also, to decrease the scores on the patients PHQ-9 and GAD-7 questionnaires. In doing so we would have less re-admissions to hospitals and a better quality of life. Our measurable goal would to have a decrease in admission to the hospital for COPD exacerbations. This would be measured by looking at how many times a patient was admitted to the hospital in the previous year and try to decrease this number. Then we would compare it to how many times she was hospitalized after educating them on relaxation techniques to see if the number of admissions decreased

Implementation Process Outcomes

           The implementation process will be that every nurse will be doing both the education and screenings by the first week once we make the change in practice. The nurse manager will ensure that all nurses are educated on the relaxation techniques and screenings, and that they are being done with every COPD admission. By the second appointment for the COPD patients, our patients will be screened on performing the relaxation techniques correctly, PHQ-9, GAD-7 questionnaires, and if the patient has been to the hospital for a COPD exacerbation. We will continue to do these screenings at every appointment. We will see our COPD patients every 3 months to evaluate. We will see them at the end of the year and compare their current numbers to their numbers in the beginning of the year. Our nurses will have quarterly meetings to ensure everyone is teaching their relaxation techniques correctly and are doing their questionnaires. Specific barriers that we could run into is nursing staff rushing through the demonstration of the relaxation techniques and rushing the patients during their PHQ-9 and GAD-7questionsairs. This will be eliminated by giving the nurses more time when checking in COPD patients.
