Completing this assessment will strengthen your understanding of how to plan and negotiate individual or group participation. This assessment MUST be satisfactorily completed to complete Assessment 4 (live face-to-face presentation of the plan). These assessments (Assessment 1 and 4) meet the three-hour clinical learning experience required in this course.

To prepare for the assessment, consider various health concerns that you would like to be the focus of your plan, the populations potentially affected by that concern, and individuals or groups in your community who may be willing to take part in a presentation about that concern and suggested strategies for health improvement.

As you begin to prepare this assessment, you are encouraged to complete the Vila Health: Effective Interpersonal Communications activity. The information gained from completing this activity will help you succeed with the assessment. Completing activities is also a way to demonstrate engagement.

Part 1: Health Promotion Plan

• Choose a specific health concern as the focus of your health promotion plan. Then, investigate your chosen concern and best practices for health improvement, based on supporting evidence.
• Identify populations potentially affected by this health concern. Determine what their related concerns may be and explain why addressing this health concern is important for health promotion.

Part 2: Individual or Group Activity Participant Recruitment

• Identify an individual or group among the affected population who may be willing to participate in an educational session about your chosen health concern and associated health improvement strategies. Then, research and document their potential learning needs and health promotion goals. Participants may include individuals, groups, or other community members.
• Contact the selected individual or group and secure their agreement to participate in the educational session.
  • Meet with the individual or group to describe the session and collaborate in setting expectations for session outcomes, establishing agreed-upon goals, and suggesting possible revisions to the plan.
  • Confirm, with the individual or group, a date and time for the educational session and document the name and contact information (e-mail or phone) of the individual or group representative.

Document Format and Length

Your health promotion plan should be 2–3 pages in length. In a separate section of the plan, identify any participants and be sure to include their contact information.

Supporting Evidence

Support your health promotion plan with peer-reviewed articles, course study resources, and Healthy People 2020 resources. Cite at least three credible sources.

Case Study, Chapter 17, Preoperative Nursing Management

1. Joan Arnold, 67 years of age, is a female patient who underwent a coronary angiogram that diagnosed severe coronary artery disease in three of her coronary arteries, the left anterior descending, the left circumflex, and the right coronary artery. She is scheduled for a coronary artery bypass graft operation tomorrow. The nurse provides patient education for Mrs. Arnold and her husband, which includes watching a hospital video on the operation, the intensive care environment, what to expect after the surgery, the recovery period, and cardiac rehabilitation. The video also covered the importance of coughing and deep breathing, using an incentive spirometer, splinting, early ambulation, pain medication, and how to position oneself safely. The video stressed the importance of not rolling onto the side that the chest tube may be in place postoperatively. The video also discussed the preoperative preparation. In my healthcare organization, there are many general healthcare technology trends we use daily. Most frequently, the use of the electronic health record (EHR), which is essentially a digital version of a paper chart (HealthIT.gov, 2018c). These EHR’s contain vital information for assessment and treatment for patients. The medical history, allergies, current medications, and treatment plans are all examples of what can be in the EHR (HealthIT.gov, 2018c). In addition to the chart containing all this information, it makes it accessible for all workers involved in the patient’s care to share information over a secure server. For example, physicians, nurses, laboratories, and pharmacies can all view vital healthcare information as well as share information among each other (HealthIT.gov, 2018c).

One potential drawback to using the EHR is patient perception. The use of the computer is vital to patient care and sometimes distances us from the patient or family members. In the last 30 years, the computer has come between this patient-centered approach to patient care (Sobral, Rosenbaum, & Figueiredo-Braga, 2015). It is widely known that a patient-centered approach brings better compliance, patient outcomes, and better patient satisfaction (Sobral, Rosenbaum, & Figueiredo-Braga, 2015). One benefit is that patients can easily access their lab and test results and communicate with physicians at home through patient portals. In the environment we live in, healthcare providers need to have the knowledge and skills necessary to recommend and navigate these digital tools (Skiba, 2017).

The healthcare trend I think is most promising, is telehealth. Just this week, my husband and I have both utilized these services with the COVID-19 restrictions on leaving the house. If someone is exhibiting symptoms of COVID-19, instead of leaving the house to see a doctor for mild symptoms, a call can be made from home and possibly prevent the spread of disease to others. Medicare has even gone so far as to expand payment for telehealth to ensure people feel comfortable using this type of technology in hopes of preventing the spread of COVID-19.
