I find it hard to separate the natural versus manmade disaster given inconvenient reality of more frequent disasters with more intensity and scope caused by man-made climate change. Sure, we’ve always had extreme weather events. But they are getting more unpredictable, more frequent, more intense, and devastating than ever before because of human activity.

Spiritual care in the face of one of these disasters may become acutely needed. While not everyone has adopted religion or subscribes to a particular faith, chaplains are trained to provide psychosocial support when needed to help communities cope with emotional needs following a disaster event (GCU, 2018). Nurses may recognize a readiness for patients to receive psychosocial and spiritual support that a chaplain can provide, even if the patient does not expressly ask for one.

Culture plays a role in the response and reaction people may display following a disaster, and the PHN should be aware of the existence of these differences and incorporate them into the nursing process when planning for care. Responses may vary widely or even seem strange to an onlooker such as a nurse, but its cultural and trauma-informed to put those preconceptions aside and meet the individual where they are in processing the disaster.

Chaplains can also help tend to the spiritual and psychosocial needs of the first responders and disaster relief teams, including nurses. Taking care of one’s own mental and spiritual well-being before helping others is an important aspect of helping during a disaster.

  • Outline of Organizational Objectives: Outline and provide a brief evaluation of specific objectives that support the organizational goals you previously identified, to include the use of an interdisciplinary approach to patient care.

  • Performance Baseline: Determine a performance baseline for the measures selected. This will enable the organization to conduct comparisons of desired goals versus actual results over time.
  • Performance Evaluation: Select one of three commonly accepted methods to measure provider quality. Summarize the features and why it applies best to the organization. Refer to the assigned reading, “The Measurement of Health Care Performance: A Primer from the Council of Medical Specialty Societies.” 
  • Definition of Success: Define what success means to the organization. Now that you have chosen measures to assess organizational performance, identify what success means to the organization; otherwise, you are chasing a moving target. Be explicit in the level of performance you see as acceptable. This will change as an organization grows, but you need to start somewhere in order to get anywhere.

This is not unlike what has occurred in the process of classifying mental disorders. A system that provides enough specificity to appropriately classify a large variety of mental disorders while also attempting to include all of the possible symptoms, many of which can change over time, is a daunting task when used by a variety of specialists, doctors, and other professionals with varied experience, cultures, expertise, and beliefs. The DSM has undergone many transformations since it was first published in 1952. Many of these changes occurred because the uses for the DSM changed. However, the greatest changes began with the use of extensive empirical research to guide the creation of the classification system and its continued revisions.

In order to assess and diagnose patients, you must learn to use the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders, usually abbreviated as the DSM-5, to render a diagnosis. In this second week of the course, you will examine how DSM-5 is organized and how clinicians use it to render diagnoses. 

Review the Learning Resources this week, with special emphasis on viewing the Diagnostic Criteria video. This video explains the purpose and organization of the DSM-5 classification system, the purpose of the ICD-10 coding system, their relationship to one another, and the importance to the PMHNP role. 

Discussion: The Psychiatric Evaluation and Evidence-Based Rating Scales

Assessment tools have two primary purposes: 1) to measure illness and diagnose clients, and 2) to measure a client’s response to treatment. Often, you will find that multiple assessment tools are designed to measure the same condition or response. Not all tools, however, are appropriate for use in all clinical situations. You must consider the strengths and weaknesses of each tool to select the appropriate assessment tool for your client. For this Discussion, as you examine the assessment tool assigned to you by the Course Instructor, consider its use in psychotherapy.
