Read this scenario: 

You have been hired as Assistant Manager of a Home Health Care Agency. The agency offers a wide range of skilled medical services such as nursing care, physical therapy, occupational therapy from qualified medical professionals. The agency also provides home health aide services including assistance with activities of daily living, such as bathing and eating.

Your supervisor told you that she recently received a call from an advocate whom an employee consulted via the Employee Assistance Program because the employee felt that she had been treated unfairly after contracting an illness. She explained that a few months ago, one of your agency’s home health aides, Sara, became ill. Sara had been coughing for approximately six weeks, lost weight without trying, had no appetite, was having difficulty sleeping, and had an intermittent fever. She became concerned and went to see a healthcare professional who diagnosed her with active tuberculosis (TB).

Sara missed a lot of time from work while completing treatment for TB. Her physician cleared her to return to work after she was no longer contagious. Upon returning to work, Sara felt isolated because her boss and co-workers refused to spend time with her. She heard that someone from the agency’s Human Resources Department told her co-workers her diagnosis. Donald Davidson, in his book, American Composition and Rhetoric, discusses the précis as follows:

“A précis is not an outline, but a summary or digest. It is useful as an exercise in grasping the essential ideas of an already completed composition and in stating these ideas in concentrated form. The précis shears away all elaborations of the thought and gives only what is left, in such a way as to make the summary a complete composition. It does not, therefore, skeletonize the original composition so much as it reduces its scale. Many of the articles in The Reader’s Digest are only précis, so skillfully done that the average reader does not know that he is reading a summary. Since the précis says a great deal within a brief space, it is of great service in taking notes on library assignments and general reading.”

The précis should be an organization of ideas, include logical sequencing of points, contain clear and meaningful expression, and use language suitable to the situation. When finished, the précis should clearly state:

  • A statement of what was studied (i.e., argued, discussed, deliberated).
  • A statement of focus of the scholarly article you selected, (i.e., how it was organized and completed).
  • A statement of what information was identified or learned from the scholarly article.
  • A statement of why the information in the article is important to your field of study.

PART II: Mind Map.

Jamie is a 3-month-old female who presents with her mother for evaluation of “throwing up.” Mom reports that Jamie has been throwing up pretty much all the time since she was born. Jamie does not seem to be sick. In fact, she drinks her formula vigorously and often acts hungry. Jamie has normal soft brown bowel movements every day and, overall, seems like a happy and contented baby. She smiles readily and does not cry often. Other than the fact that she often throws up after drinking a bottle, she seems to be a very healthy, happy infant. A more precise history suggests that Jamie does not exactly throw up—she does not heave or act unwell—but rather it just seems that almost every time she drinks a bottle she regurgitates a milky substance. Mom thought that she might be allergic to her formula and switched her to a hypoallergenic formula. It didn’t appear to help at all, and now Mom is very concerned.

Cases like these are not uncommon. The mother was concerned and thinking her daughter may have an allergy; she changed to a different formula. However, sometimes babies have immature GI tracts that can lead to physiology reflux as they adapt to normal life outside the uterus. Parents often do not consider this possibility, prompting them to change formulas rather than seeking medical care. As in the case study above, GI alterations can often be difficult to identify because many cause similar symptoms. This same issue also arises with adults—adults may present with symptoms that have various potential causes. When evaluating patients, it is important for the advanced practice nurse to know the types of questions he or she needs to ask to obtain the appropriate information for diagnosis. For this reason, you must have an understanding of common GI disorders such as gastroesophageal reflux disease (GERD), peptic ulcer disease (PUD), and gastritis.

  • Review Chapter 35 in the Huether and McCance text. Identify the normal pathophysiology of gastric acid stimulation and production.
  • Review Chapter 37 in the Huether and McCance text. Consider the pathophysiology of gastroesophageal reflux disease (GERD), peptic ulcer disease (PUD), and gastritis. Think about how these disorders are similar and different.

Preparing for disasters is something many only think about during “that season”; hurricane season, winter blizzards, tornadoes , fire, flooding etc. however, disasters can happen at any time, so continued and sustained preparedness is essential.

       For the most part I knew my level of preparedness as I’ve lived in many areas of the world where “that season” happened on a regular basis.  Essential documents stored in plastic bins, easily accessible to place in to a car in the event of needed evacuation or sheltering in place, food, water, medical documents as well; only once I was not prepared but many things were happening in my life at that time which caught me off guard.  I worked a disaster relief effort with the American Red Cross in the aftermath of Hurricane Andrew in southern Florida and learned many things due to that experience and my time in the military has prepared me for many other man-made disasters.

       We have recently moved to the southwest and I now own a home again, so we are not quite as prepared as we were previously as a homeowner.  The tools and supplies needed to secure my home at this time are lacking so this is a step in preparation that must be accomplished, otherwise, based on the checklist, I am still prepared for nearly any disaster that may strike, or at least I would hope to be! 

       From the perspective of a community health nurse; education is the key to disseminating information to community members.  From participating in and education regarding local flu outbreaks, encouraging vaccinations for those at risk or simple education regarding hand hygiene and the correct way of covering your cough, along with health and wellness to maintain the highest levels of immunity for something as simple yet as deadly as the flu can be, is important.  Man made, natural or environmentally fueled disasters are difficult to disseminate information except through ongoing education or community preparedness sponsored events or work shops.  Ongoing drills within the school systems or community centers will assist in preparedness for disaster specific to the geographical area.  Instructions; written, verbal or expressed for personal preparedness, the how and why, and where to find needed supplies would all begin and continue through education.  Reporting of potential outbreaks or disasters through proper channels would also provide the community with added resources.  In the event of a disaster, nurses would need to assist in identifying who is at risk and what to do regarding those individuals, their friends and families, as would the nurse.  But always keeping in mind the scope of practice and current knowledge base of the nurse.
