Based on Meilaender’s (2013) perspective of suicide, it is morally wrong because there is an unwillingness to receive the life given, by our creator, moment by moment.  Suicide
is not God’s will, it is the irrational desire of a man to be in control and a repercussion of sin with man acting as the creator, instead of the created.  Meilanender (2013) contends that life is not “our” possession, nor ours to take whether by suicide or euthanasia.  I appreciate his comparison of our lives to being characters in a story that God created while God gives us the freedom to act according to the nature he provided.  However, I don’t believe that it was ever God’s intent to allow us the freedom to rationally take a life by suicide.

I have a personal experience with suicide, so I have struggled with the morality, or lack thereof.  Where Meilaender (2013) states that the sin of suicide dies with the
person and they are not necessarily damned is relevant for all suicides and euthanasia because God judges the person, not the individual deeds.  Meilaender (2013) also pointedly asserts that the autonomous decision to end a life, whether it be a “right to die with dignity” state or not, there is an inherent risk that it is still morally wrong
and judged by God as a “lordship” over another human’s life, which is biblically wrong.  

I am not a hospice nurse for a reason.  I know that I would struggle with the person begging me to give them an additional dose of morphine to end their pain and
misery.  This is a terrible predicament for a nurse who is caring for their patients and family members both physically and emotionally.  Meilaender (2013) claims that hastening one’s inevitable death is a form of abandonment.   I know I would struggle with temptation to bring relief to someone who is severely suffering, so I choose to not put myself in that position.  My compassion would cause an internal struggle that I can avoid and a judgement that I, as a sinful human being, do not want to make, even if God does not judge me for individual deeds.

Suicide and euthanasia are extremely sensitive topics for the majority of individuals including health care workers. According to the Christian belief, it is considered a sinful act and therefore goes against Christian beliefs. Meilaender (2013) states that “Christians have held that suicide is morally wrong because they have seen it in a contradiction of our nature as creatures, an unwillingness to receive life moment by moment from the hand of God without ever regarding it as simply “our” possession” (Meilaender, 2013). He goes on to say that our life is not something we own and that by committing suicide we are essentially playing the role of creator. We need to remember that life is a gift from God and to take one’s own life is a selfish act and disrespectful to God for the gift of life he gave to us.

Suicide and euthanasia are controversial topics. I don’t whole heartedly agree with Meilaender. I believe that if a patient has a chronic illness that is causing them to have a very poor quality of life and/or chronic pain, then that person should be allowed to decide if they want to carry on with life living that way. What a bleak existence it would be. My husband and I have had numerous discussions about this very topic. We both agree that if either one of us were extremely ill, we should find a way to put that person out of their misery. On the flip side, if a healthy person were to take their own life, then I totally agree with Meilaender and that person is selfish and not following in the footsteps of God

Autonomy is something that should be respected. When a person is going to get any invasive medical procedure or treatment done it is the responsibility of the healthcare provider to explain and make sure the patient is aware of the risks of the procedure. The healthcare provider should answer any questions that patient may have. Once there has been understanding of the procedure including the pros and cons, the patient is to sign an informed consent. When the patient signs the consent they are giving authority to accept medical treatment (Rock & Hoebeke, 2014).

            There may be barriers that can interfere with the rights of a patient when it comes to informed consents. If a patient does not speak English it can interfere with understanding of the procedures and they may not fully comprehend why they are signing a consent (Chesnay & Anderson, 2016). When situations like these arise, hospitals can provide a professional translator to assist. There can also be cultural barriers where patient’s culture or beliefs does not allow them to go forth with a procedure that may improve their quality of life. However, the healthcare provide should respect that patient’s culture and beliefs. It is crucial that consents are understood and signed by the patient before any treatment is given or else the healthcare provider can be charged with assault. Physicians need to explain the treatments and nurses should be able to confirm the patient understands. It is very important to protect the right of the patient which is the right of self-determination. That is why the patient should have full understanding of what treatments consists of, the pros and cons, and the risks of not doing the treatment. Once that is done they need to sign a consent to confirm that they understood and made a conscious decision to accept treatment.

As our lecture describes, “death is a particularly traumatic and difficult experience for both family and caregivers” (Grand Canyon, 2015). A rather interesting distinction I observed while working on both of these units and witnessing death in different underlying situations/stages: death on the oncology unit usually came after a long, hard fight with cancer. As such, patient’s families were often understandably distraught and through their grief expressed feeling “robbed” of time with their loved one. When death occurred on the geriatric unit however, family members would sometimes express feelings of gratitude that their loved one lived to a ripe age and felt at peace that their time had come, as it was expected. In witnessing these experiences, it made me understand that death is easier to swallow/digest for some when it occurs from natural age process versus caused by disease.

The acceptance of death and the grieving process is experienced differently by everyone. Personally, I feel my bedside experience as a nurse has helped me understand this process a great deal and I empathize greatly with the patients/families I have encountered.

I have only witnessed death a couple of times at work. I have worked with hospice patients that visit the doctor’s office and these have been about comfort. Since I am a float in the clinic setting, I do not come across much death. I did when I worked on the medical floor; there, acceptance of the death depended more on if it was expected or not. I think that when we know death is going to occur it is easier to accept and come to terms with. When death is not expected it can be harder because of the sudden loss. I do not think that the lack of experience at work has shaped my view on death because I have dealt with it personally.

I think that I handle death fairly well because I grew up with death being a natural part of life. As we grew up on the farm it was normal to lose animals. We also have a very large extended family and we went to funerals fairly regularly. While others have viewed that as wrong or depressing to have children go to funerals, it was never viewed that way by my family because it was a part of life and we celebrated that life. I have not shielded my children from death either. My sister-in-law and I have different views on this; I accept her decision, she does accept not mine. She was upset when we brought our children to the first funeral on my husband’s side of the family but it was very natural to me. Death is only one step in life and should not be feared.

Acute kidney injury has three categories, they are prerenal, postrenal, and intrarenal/intrinsic.  The first category, prerenal, means that the injury to the kidney occurs before the kidney itself.  This type of injury is caused because of reduced perfusion of the kidney. This reduction can be caused by absolute decrease in circulating volume caused by hemorrhage, dehydration, or burns. It can also be caused by relative decrease in circulating volume caused by, distributive shock, third-spacing and edema, or decreased cardiac output.  Another cause can be renal abnormalities such as occlusion or stenosis of the renal artery.  The second category is postrenal, this can be caused by obstruction of normal outflow of urine from the kidneys.  Things that can cause this are: benign prostatic hyperplasia, kinked or obstructed catheters, intraabdominal tumors, strictures, or calculi.  The third category is intrarenal/intrinsic, this involves either vascular, interstitial, glomerular, or tubular.  If it is vascular, it can be caused by vasculitis, emboli, or nephrosclerosis (due to primary hypertension, hypertensive emergencies, and urgency).  If interstitial, it can be caused by acute allergic interstitial nephritis or acute pyelonephritis.  If glomerular, it can be caused by acute glomerulonephritis.  If tubular, it can be caused by ischemic issues such as prolonged prerenal failure, transfusion reactions, or rhabdomyolysis.  Tubular can also be caused by nephrotoxic issues like prolonged post renal failure, certain antimicrobials (antibiotics, antifungals, and antiviral drugs), radiographic contrast media, certain cytotoxic chemotherapy agents, recreational drugs, environmental agents, and snake and insect venom.

Obesity is measured by a screening tool measuring one’s body mass index (BMI). Body mass index is measured by your height and weight.  A BMI of 30.0 or higher means a person is obese. Being obese is an unhealthy weight that is over what would be normal.

What health risks associated with obesity does Mr. C. have? Is bariatric surgery an appropriate intervention? Why or why not?

Mr.C’s health risk includes heart disease, diabetes, cancer, respiratory issues, physical immobility issues, and mental issues. Premature death. (1)

If Mr.C has done everything to try and lose weight on his own and nothing has worked, then I believe bariatric surgery would be an appropriate intervention. Some patients don’t even try to lose the weight the correct way and go for the easy way out. Complications of having bariatric surgery will pose problems when the patient continues to live this unhealthy life style post-surgery. Bariatric surgery may improve quality of health. It may treat or control diseases such as diabetes, hypertension, and heart disease (2).
