You will also develop a plan of care for that disease or illness.
 

In your analysis, you will describe the pathophysiology for your selected disease or illness, propose historical explanations for variations in findings (when applicable), and demonstrate use of nursing conceptual models to frame your discussion of adaptation and stressors collectively for system analysis.
 

Your plan of care should utilize the PIE (planning, intervention, and evaluation) format. The planning (P) section should include at least two short-term and two long-term goals that are most appropriate for your chosen case, and each goal should have measurable criteria and have a hypothetical target date or time. The intervention (I) section should include interventions or nursing actions that directly relate to the selected case’s goals and that are specific in action and frequency. The number of interventions should be appropriate for helping to meet individual goals. Finally, the evaluation (E) section of the plan of care should include proposed measures for determining success of the plan and an evaluation of the potential success of the plan, based on those measures. 

    

Specifically, the following critical elements must be addressed:

      literature.
       
    2. b)  Identify physiological stressors that may affect the course of the disease or illness, and explain their impact.
       
    3. c)  Identify adaptive physiological mechanisms that may affect the course of the disease for your selected case.
       
    4. d)  Assess the historical impact of patient care technologies on patient outcomes for your selected case. Be sure to justify your claims with scholarly

  3. Planning for care for your selected case
     
    1. a)  Write a goal statement that is patient-centered and contains at least one measurable criterion or target date/time. In order to address this
      element comprehensively, you should be sure to provide at least two short-term and two long-term goals for your selected case.
       
    2. b)  Analyze the critical nursing concerns associated with this disease state, and prioritize them from a treatment standpoint.

  1. a)  Identify the most appropriate measures for determining the success of your plan of care. In other words, what data will you need to collect to

A variable is something that varies or changes. “A variable in research simply refers to a person, place, thing, or phenomenon that you are trying to measure in some way” (University of California Libraries, 2019, para. 5). Independent variables are stable and unaffected by other variables you are attempting to measure. These are often manipulated by the researcher. Dependent variables are those that depend on other factors and change as a result of an experiment. This is the response that is measured. Dependent and independent variables are important because they drive the research process. A variable as opposed to a constant is simply anything that can vary and that many researchers consistently look at the relationship between the two variables. In order to evaluate my EBP project, I will have managers audit charts to determine if education provided to the patient was effective by not being readmitted with the same diagnosis. A few independent variable are patient’s willingness to learn, age, and time. These are important factors to consider when performing the audits. Some patients are not willing to make a change to their health and are set in their ways. The time spent with the patient is also important to consider. If the patient is being discharged and is ready to go, they aren’t going to fully be attentive to the teaching as they are eager to leave. Timing is key and having enough time to perform the teaching is important as well. Some dependent variables to consider are the patients who are admitted with stroke and chest pain who are tobacco users. The outcome depends on the effectiveness of the teaching performed by the nursing staff. The variables need to be collected because they will affect the evaluation. Frequently when the shortage of primary care providers is mentioned with regard to healthcare reform, insurance companies and other institutions forget to factor in the impact nurse practitioners can have on increasing access.     As listed in the articles provided, there are many barriers to health care access for patients to receive primary care services from a family nurse practitioner. Among them are the annoyingly-varied scope of practice regulations that are different from state-to-state, health care reimbursement from insurance companies, misinformation regarding the roles of the nurse practitioner in health care, and the unvoiced stigma towards nurse practitioners that might exist (Bernazzani, 2016). These are all factors that impede our ability to practice as primary care providers. These barriers do not decrease the burden on primary care providers, despite the fact that more nurses are graduating from these graduate programs to become nurse practitioners. I believe that if the scope of practice for family nurse practitioners can be more standardized across the United States, rather than varying state-by-state, it would have a huge positive impact on the other barriers. A standardized scope of practice for a family nurse practitioner may reduce confusion regarding the role the FNP has in primary care (Altman, 2016). This will allow easier collaboration between other primary care providers. In addition, reimbursement from insurance companies may be better able to recognize the FNP as a primary care provider. If scope of practice is standardized, perhaps patients can accept care in an easer fashion from an FNP and may promote better access to health care.

· Select a patient factor different from the one you selected in this week’s Discussion: genetics, gender, ethnicity, age, or behavior. Think about how the factor you selected might impact the pathophysiology of CVI and DVT. Reflect on how you would diagnose and prescribe treatment of these disorders for a patient based on the factor you selected.

· Review the “Mind Maps—Dementia, Endocarditis, and Gastro-oesophageal Reflux Disease (GERD)” media in the Week 2 Learning Resources. The purpose of the Course Project is to give the student the opportunity to apply concepts in transcultural nursing by performing a transcultural nursing assessment.

The purpose of Milestone 1 is to identify an adult who is not a relative who is willing to participate in the transcultural assessment and to secure the instructor’s approval for the assessment. Note: The actual assessment does not occur until Milestone 3. It is not a purpose for this assignment.

Requirements and Guidelines

3. Identify a potential individual who has been living in a culture other than the one of origin for at least 2 years. This individual must be born outside the United States. The individual can be a patient, friend, or colleague, such as a physician, or acquaintance, but cannot be a relative. The individual must be living in your community and must be currently receiving or have previously received healthcare services in your community. The individual’s race, ethnicity, language, religion, and culturally based beliefs about healthcare and illness should be quite different from yours. The individual must speak your language. An interpreter is not permitted for the actual assessment, which occurs in Milestone 3 (Week 6). 
Note: This is the same individual for whom you will develop verbal assessment questions in Milestone 2 and whom you will ask those questions in Milestone 3 and then reflect on your learning about the Course Project and the assessment.

4. Individuals can come from a variety of settings. Recent immigrants, migrant workers, exchange students, international students, long-term care residents, refugees, coworkers, physicians, patients, employees at local ethnic businesses, religious venues, community centers, and individuals who have recently moved to your community are some suggestions. The individual does not need to be a healthcare worker but must have received healthcare services in the country of origin as well as since arriving in the United States. Work with your instructor prior to submitting Milestone 1. Approval must be provided by your instructor before submitting Milestone 1 (this assignment).

5. Speak with the individual, and obtain permission to conduct an assessment later in the course. Clearly explain that statements made in the assessment will be recorded (written) and submitted to the instructor only. It is not intended for public distribution. Anonymity will be maintained by using first and last initials only. Do not use names in any of your documents.

6. Download the NR394_Individual_Profile_Template (Links to an external site.). Although you are not performing the official assessment until Milestone 3, it will be necessary to obtain some general information about your individual to complete Milestone 1. This information may come from the individual or Internet sources.

7. Fill in information about the potential individual using the template. You are expected to write clearly and use proper grammar and spelling and eliminate typos.
