LEVELS OF EVIDENCE FOR ECONOMIC EVALUATIONS

Levels

1. Decision model with assumptions and variables informed by systematic review and tailored to fit the decision making context.

2. Systematic review of economic evaluations conducted in a setting similar to the decision makers.

3. Synthesis/review of economic evaluations undertaken in a setting similar to that in which the decision is to be made and which are of high quality (comprehensive and credible measurement of costs and health outcomes, sufficient time period covered, discounting, and sensitivity testing).

4. Economic evaluation of high quality (comprehensive and credible measurement of costs and health outcomes, sufficient time period covered, discounting and sensitivity testing) and conducted in setting similar to the decision making context.

Psychiatric Nurse Practitioners, have successfully increased access to care in underserved rural communities by practicing at the fullest extent of their scope without mandated supervision, utilizing telepsychiatry practice, while expanding PMHNP rural mental health education and research to meet and absolve pressing rural mental health challenges (Finley,2020).

Current evidence supports that rural mental health care improves when PMHNPs have full scope of practice, utilize telepsychiatry, engage in related scholarly, and have formalized education and training for rural health care delivery, which collectively answer the professional and moral call serving the underserved rural population with mental illness (Finley,2020). I chose to become a Mental health Nurse Practitioner, because it’s a passion of mine. I live in a rural area and would like to create a program for this area and the inner city where I also work. As a nurse practitioner I feel I can teach, educate and help each area, even though the needs are different. My heart goes out for the mentally challenged.

Database

I recommend, Thoreau advanced search database in the Walden Library, so far, I have had a lot of luck with this database. I have used different search engines before however, Thoreau advanced search is a miracle worker for me. Thoreau, searches most of the library’s collection in one place. It includes articles, books, videos, peer reviewed journals and more. I could not name them all however, Thoreau search so many databases, I don’t have to scroll down and figure which to choose it does the work for me.

Mary is a 35 years old electrical engineer who presents to the office for evaluation of a rash on her face that has been present for 1 week. She denies new soaps, detergents, lotions, environmental exposures, medications, and foods. The rash is across her face and the bridge of her nose. She states that she first noticed it after spending a week hiking and camping in the Appalachians. The lesions itch and are painful. She has not tried anything to make it better, but she has noticed that going outdoors makes it worse. She denies any spread of the rash to other areas. She has never had this rash before.

She has noticed some increased fatigue, fever, and weight loss. She denies headache, sore throat, ear pain, nasal or sinus congestion, chest pain, shortness of breath, cough, abdominal pain, and pain with urination, constipation, or diarrhea. She does have mouth soreness. She has noticed some increased muscle aches and pains, which are worse in the hand and wrist. She denies early morning joint stiffness or difficulty with being able to move in the morning. She denies temperature intolerance, polyuria, polydipsia, or polyphagia.

She had a tonsillectomy at age 9 for chronic strep throat infections. She has been healthy as an adult. She has never had children. She has never been hospitalized for any reason.

Her family history is significant for a mother with rheumatoid arthritis. Her father is healthy.

She does not smoke; she drinks a glass of wine nearly every night with her dinner; she denies illicit drug use. She completed a master’s degree in engineering. She has lived with her boyfriend for the past 5 years.

Patient is an alert young woman, sitting comfortably on the examination table. BP 112/66 mm Hg; HR 62 BPM and regular; respiratory rate 12 breaths/min; temperature 100.3°F. Several erythematous plaques scattered over the cheeks and the bridge of nose, sparing the nasolabial folds. Normocephalic, atraumatic. Sclera white, conjunctivae clear; pupils constrict from 4 mm to 2 mm and equal, round, and reactive to light and accommodation. Oropharynx moist with erythema in the posterior pharyngeal wall; no exudates; shallow ulcers in the buccal mucosa bilaterally. Neck supple without cervical lymphadenopathy or thyromegaly. Full range of motion; no swelling or deformity; muscles with normal bulk and tone.

For this assignment you are required to sit down for a conversation with someone whose religious identity differs from your own (this means that if you identify as Christian, for example, you should find a conversation partner who does not identify as such). The purpose of this exercise is for you to engage with someone else in conversation. That means that you will not only ask questions, but you will hopefully answer questions about your own beliefs, as well.

Review the article for this activity, “Talking About Religion – How to Do It Right” (link in Required Resources), for some guidance on how to engage in conversations about religion before meeting with your partner. You may also wish to share this information with your partner.

In this exercise, you will listen to the ideas of someone who you identify as religiously “other.” You will share your own ideas and report on what you learned. This would include what you learned about your conversation partner’s beliefs, and how those beliefs compare to your own religious upbringing and/or current practice.

The report should give a description of the major topics of discussion and a detailed summary of what you learned. As part of your assignment you should include the following:

• Describe your conversation partner’s beliefs. Some things to consider include the following:
  • What are the material expressions of their tradition (statues, paintings, music, etc.)?
  • Do they see any conflict between the stated beliefs of their tradition and their own beliefs?
  • Based on your partner’s description, which theory of the origin of religions (Week 1) offers the best explanation for their approach to religion?
  • What challenges can your partner identify that their religious tradition faces in the modern world? Do they feel that their tradition is responding positively?
  • Is the tradition focused on belief (orthodoxy) or behavior (orthopraxis)?
  • Apply the 8 elements of religion from Week 1.
• Provide some cultural and historical context for the religious tradition of your conversation partner. (It is important that you do some research before you have your conversation so that you are able to ask informed questions that come from genuine interest.) Make sure you use citations in your discussion of what you learned from your research.
• Compare your partner’s beliefs and practices to your own religious upbringing and/or current practice. This is to be done in the report not as a debate with your partner.
• Conclude with your personal reaction to this experience and any additional questions that came up after your conversation. Be kind!

This is a formal academic paper so pay careful attention to the basics of writing a good English composition.
