Purpose

The purpose of the Course Project is to give the student the opportunity to apply concepts in transcultural nursing by performing a transcultural nursing assessment.

The purpose of Milestone 1 is to identify and describe an adult, non-relative, born outside the US, whose culture is different from yours and is willing to participate in a transcultural nursing assessment during Wk. 6. (Milestone 3).

Course Outcomes

CO1: Identify theories, concepts, and beliefs related to transcultural nursing. (PO1)

CO2: Communicate effectively with culturally diverse individuals, families, and organizations. (PO3)

CO4: Perform a culturally competent nursing assessment. (PO7)

Notes: 

• Send your instructor a brief email during Wk. 1 or 2 briefly describing the individual’s place of birth, age and current city/community of residence.
• The actual assessment does not occur until Milestone 3. The assessment is not a purpose for this assignment.

Requirements and Guidelines

1. Review the Course Outcomes for this assignment.
2. BEFORE beginning this Milestone, make sure you have received “approval” of the individual from your instructor. This must be done during Wk. 1 or 2 of the course. Send your instructor a brief email with the initials of the individual, country of birth, current city/community of residence. Your instructor will reply to your email letting you know it is all right to proceed with Milestone 1 (“approval”).
3. Review the guidelines for Milestones 1, 2, and 3, paying particular attention to Milestone 1 for this assignment. The guidelines for all of the Milestones are located on the left-hand navigation menu. Also, read the Course Project Overview document located under the Modules Tab. Here you will find more details and helpful tips.
4. Identify/describe the individual approved by your instructor. This individual:
   • Has been living in a culture other than the one of origin for at least 2 years.
   • Was born outside the United States.
   • Can be a patient, friend, or colleague, such as a physician, or acquaintance, but cannot be a relative.
   • Must be living in your community and must be currently receiving or have previously received healthcare services in your community.
   • Race, ethnicity, language, religion, and culturally based beliefs about healthcare and illness should be quite different from yours.
   • Must speak English. An interpreter/translator is not permitted for the actual assessment, which occurs in Milestone 3 (Week 6). 

Note: This is the same individual for whom you will develop verbal assessment questions in Milestone 2 and whom you will ask those questions in Milestone 3. Then you will reflect on your learning about the Course Project Milestones, assessment and how your future practice will change based on your learning.

1. Individuals can come from a variety of settings. Recent immigrants, migrant workers, exchange students, international students, long-term care residents, refugees, coworkers, physicians, patients, employees at local ethnic businesses, religious venues, community centers, and individuals who have recently moved to your community are some suggestions. The individual does not need to be a healthcare worker but must have received healthcare services in the community of origin as well as since arriving in the United States.
2. Speak with the individual, and obtain permission to conduct an assessment later in the course. Clearly explain that statements made in the assessment will be recorded (written) and submitted to the instructor only. It is not intended for public distribution. Anonymity will be maintained when you use first and last initials only on all templates. Assure the individual that the assessment (Milestone 3) should not take more than one hour of the individual’s time.
3. Download the NR394_Individual_Profile_Template. (Links to an external site.) Although you are not performing the official assessment until Milestone 3, it will be necessary to obtain some general information about your individual’s background to complete Milestone 1. This information may come from the individual or Internet sources. Basically, you are comparing the individual’s previous and current communities/cultures.
4. Type information about the potential individual using only the template provided. You are expected to write clearly and use proper grammar and spelling and eliminate typos.
5. Submit the completed template by the end of Week 3 at 11:59 p.m. MT.
6. Contact your instructor if you have any questions.

Example: KC was born in Hwidiem, Ghana, a small village in Africa, where he did not graduate from high school. He is 72 years old. He immigrated to the United States at age 28 and has lived in Grove City near Columbus, Ohio since then. Hwidiem is considered the provincial capital (fill in information about the community or city as you can find). He was married but divorced after a traumatic brain injury. Since then, he has had many part-time jobs and studies his Bible faithfully. Grove City is a town in Ohio (fill in information about the city or community). Currently, he receives healthcare for his hypertension control. I met him 3 years ago at a senior citizens’ center where I volunteer.

Initial Post Instructions
The principle of utility involves maximizing happiness as a desirable outcome of decisions. Although it does not get directly said, there is an inverse intention to minimize the undesirable outcome of disaster. Utilitarian decisions are directed toward outcomes—that is, the consequences of decisions.

We need to look at results. We first look at the actual results of an action. We judge if it was the best possible result. We can judge the actual results in comparison to other results that reasonably could be said to have been possible.

If we do not yet have the actual results of an action, we do not know if it is moral or not. We can talk hypothetically about what might happen, and then what that would show about the morality of an action. However, if we do not know what the action had as its consequences, we cannot yet say if it is moral or not.

Initial Post Instructions
For the initial post of this week’s discussion respond to one of the following options, and label the beginning of your post indicating either Option 1, Option 2, or Option 3:

Option 1: You are a nurse on a floor with only elderly patients. Every day, each patient tells you about how much pain they are in and asks you to help them. They want you to inject them with something to end their lives. If the patients die, the beds on that floor would be freed up for other patients. The hospital is at 100 percent capacity. There is no other hospital for 30 miles. Other patients may be not receiving care due to a lack of free beds. What is the moral thing to do here? Why is that the moral thing to do? What would an utilitarian say is the moral thing to do? Why would they say that? Compare and contrast the utilitarian approach with that of an ethical egoist or social contact theorist

Option 2: A new social media app is offering itself to you for free. If you upload a picture to it, the app will show how you will look at 10 years. John Doe, a friend of yours, says not to use the app as it will then possess your biometric facial data. Jane Doe, another friend of yours, says that she heard the app shares the facial data with a security firm that helps the government detect terrorists at airports. Should you use this app? Why or why not? If John Doe is right, would an utilitarian say it is right to use the app? Why or why not?  If Jane Doe is right, would a social contract theorists say it is right to use the app? Consider the role the Fourth Amendment at play here.

Option 3: You are a nursing student at the XYZ College. It has a 50 percent acceptance rate (half the applicants do not get in). XYZ is a public college. XYZ has decided to implement an affirmative action policy. The college has few students over the age of 50. To encourage more students of that age, every student 50 or older will receive a bonus point. A student’s admission is dependent on having 11 points. One earns points for a GPA above a certain score, ACT/SAT score above a certain number, having a letter of recommendation, etc. XYZ also lacks LGBT students, Muslim, and African-American students and is considering offering a bonus point for any student fitting those categories. What is the key moral conflict for XYZ? What social values should XYZ promote here? What diverse populations are involved here, and what are their interests? Do you think XYZ’s social action is the correct solution to lack of diversity? Why or why not? Factor the ethics of egoism and utilitarianism into your answer.

Prepare a 3–4-page report on nurse-patient communication in which you address types of communication, factors that influence the communication experience, and how the patient experience affects health outcomes. Recommend evidence-based strategies to improve nurse-patient communication and explain how the strategies consider patient demographics.

Nursing professionals do not only communicate with others within their organizations—often, they are also the first point of contact with patients. Thus, nursing professionals need superior communication skills to explain procedures and medication instructions, as well as to listen to what patients need to say.

Imagine that, in an effort to reduce the number of patient complaints about nursing staff, organizational leadership has asked your department to prepare some preliminary research on a number of different topics. Your topic is patient communication, such as the following:

• The types of communication that take place between nurses and patients.
• The factors that can influence both positive and negative experiences.
• How the experience can impact health care outcomes.
• Strategies that can improve the communication between nurses and patients.

Preparation

Search  the Internet for peer-reviewed journal articles or other professional resources on the topic of effective communication with patients. You will need at least 3 resources to support your work in this assessment.

Directions

Prepare a report on patient-nurse communication in which you include the following:

• Describe the types of communication that take place between nurses and clients/patients. Do not forget to consider types of communication beyond verbal and written on paper.
• Explain the factors that influence positive and negative experiences during the communication process. (Hint: Consider the experiences of both the client/patient and the nurse. Also, consider things such as age, gender, culture, and so on, of both the client/patient and the nurse.)
• Explain how the client/patient experience can impact health care outcomes.
• Recommend evidence-based strategies for improving communication between client/patients and nurses.
• Explain how the strategies consider different client/patient demographics.

Format this assessment as a report that you would give to your supervisor. It may be helpful for you to review how your organization formats internal reports and incorporate your findings. You are still required to adhere to APA guidelines for in-text citations and references, as well as for formatting your reference page.

Additional Requirements

• Include a title page and a reference page.
• Ensure your assessment is 3–4 pages in length.
• Use double-spaced, 12-pt., Times New Roman font.

The outline can be presented in a bullet point, numbered, or lettered format. Please include the method of delivery, intended audience, introduction, main body, conclusion, and the three references- to include any updated resources based on your findings since week 3.
