I remember when I was in nursing school there was a clinical instructor that everyone talked about and no one wanted to be in her clinical group. My second semester I ended up in her group and because of what everyone had said about her I was very nervous and anxious to be in her group. She was very hard because she expected each of us to know our drugs we would be giving for our patients down to the smallest detail and what their medical conditions were and why exactly they were in the hospital. It all seemed like a lot to remember or memorize the day before clinical and she always asked each of us questions before, during and in conference after each clinical day.
It became a great experience when I understood that she was only trying to get us to critically think everything through and that she wasn’t out to fail us. What she expected from us was that we participate and try to give her reasonable answers, she never belittled us when we were wrong, only provided answers. She motivated me to challenge myself without fear of failure. I enjoyed learning from her so much that I requested her again when I found out she was teaching clinical again in my fifth semester.
I feel that the cognitive learning theory best describes the method by which I learn. “According to this perspective, the key to learning and changing is the individual’s cognition (perception, thought, memory, and ways of processing and structuring information). Cognitive learning—a highly active process largely directed by the individual—involves perceiving the information, interpreting it based on what is already known, and then reorganizing the information into new insights or understanding” (Bastable, 2013, p. 73). As a very visual learner I absorb what I see and hear in order to process it and be able to understand and use it in my practice.

I viewed the documentary titled “The Invisible Patients documentary” and an ethical dilemma that was experienced by the nurse practitioner is the decision about hospice care. The patient has a right to decide how they want their life to end in regard to resuscitation, and it is one of the nurse practitioner duties to educate the patient on what a do not resuscitate (DNR) means and the accept their terminal prognosis or to have do-not-resuscitate (DNR) order (Bhatnaga & Lagnese, 2019). Patients should be allowed to receive all the care needed until the end of their life. The nurse practitioner in the video appears to be giving her personal views and wanting to listen to the patient and his wishes. Roger verbalizes his desire to live even on a respirator and it is not the duty of the nurse practitioner to discourage the patient from the right to desire life.
The nurse practitioner in the documentary as a disruptive innovator was her desire of delivering and have more nurses in primary care because they reach patients that are in unfortunate situations and needs the help of the nurse practitioner to provide care and advocate on their behalf whether by calling doctors or coordinating care on their behalf. An awareness of personal values generates more consistent choices and behaviors; it can also assist APNs to be aware of the boundaries of their personal and professional values so that they can recognize when their own positions may be unduly influencing patient and family decision making (Hamric, 2014). The nurse practitioner should separate their personal beliefs from situations that where an ethical dilemma is involved and follow the nurses code of conduct to make appropriate decisions.
Overcoming barriers when faced with ethical dilemma is being an active listener with good communication skills. We must listen to the patients concern and wishes and communicate with them at a level that is understandable and open to answer questions with clarity.

2-The American Association of Colleges of Nursing proposed the clinical nurse leader (CNL) role in 2007 as their response on how to manage the needs of an aging population and improve the nursing profession. When the CNL position was first proposed and implemented in 2008, candidates to be a CNL had to have a baccheluers degree with a minimum of two years nursing experience and be well respected by peers and physicians. They were expected to oversee units with 12-18 patients, perform daily rounds, serve as a resource for nurses, review patient outcomes, and teach change management and evidence based practice to nurses (Sotomayor, 2017).
In units where a CNL was present, there was shown to be a significant reduction in patient falls, CAUTIs, central line-associated infections, and hospital-acquired pressure ulcers (Sotomayor, 2017).
A CNL influences direct patient care by assessing how nursing care is currently being delivered and how it can be improved. They are responsible for educating nurses on their unit regarding how improvements can be made and also tracking how effective these changes are.
This role is not only reserved for inpatient units but can also be used for nursing positions out in the community to improve access to care and care coordination.

3-Thank you for sharing your findings with the rest of the class. It was a great post. I actually have clinical nurse leaders at my facility and I find them to be extremely helpful, bot only to the nurses but the patients as well. They are familiar with policies and procedures and serve as a great resource when needed. As I new nurse I am always looking for a clinical nurse leader. My charge nurse is a great resource. Following the PICOT format, write a PICOT statement in your selected practice problem area of interest, which is applicable to your proposed capstone project.
The PICOT statement will provide a framework for your capstone project (the project students must complete during their final course in the RN-BSN program of study).
Conduct a literature search to locate research articles focused on your selected practice problem of interest. This literature search should include both quantitative and qualitative peer-reviewed research articles to support your practice problem.
Select six peer-reviewed research articles which will be utilized through the next 5 weeks as reference sources. Be sure that some of the articles use qualitative research and that some use quantitative research. Create a reference list in which the six articles are listed. Beneath each reference include the article’s abstract. The completed assignment should have a title page and a reference list with abstracts.
Suggestions for locating qualitative and quantitative research articles from credible sources:
To narrow/broaden your search, remove the words qualitative and quantitative and include words that narrow or broaden your main topic. For example: Diabetes and pediatric and dialysis. To determine what research design was used, review the abstract and the methods section of the article. The author will provide a description of data collection using qualitative or quantitative methods.
