anna is a 29-year-old G2P1 who has just had a 20-week routine anatomy ultrasound. The physician informed her that there were some “soft markers” for Down syndrome and that genetic amniocentesis is available if she wants to have it. Please answer all questions Anna asks if the ultrasound shows that the fetus has Down syndrome. How can the nurse explain the “soft markers” for Down syndrome? What counseling should Anna receive about amniocentesis? Anna decided to go home and discuss the findings with her family. She ultimately decided to have genetic amniocentesis and is back today for that appointment. What are the nursing implications of genetic amniocentesis? The results of the amniocentesis show trisomy 21. After genetic counseling, Anna chooses to continue the pregnancy. How can the nurse help Anna at this time in her pregnancy?

PLEASE COMMENT ON THIS. WHAT ARE YOUR THOUGHTS? Older people need to be given proper care. Many older people are neglected and left to live on their own, or a caregiver is employed to stay with them by their children. In this paper I will discuss an older woman’s perspective on the current dwelling place, specifically bathroom, cooking, sleeping, and living areas, in terms of safety concerns. Also, I will identify the transformation that can be made to improve home safety. Lastly, I will give my opinion on the changes made whether they have improved my safety. I live in a four-bedroom house alone. Currently, I am 65 years of age. The house which I am living in is fully furnished because my children always purchase the equipment regularly. I usually use a gas cooker when preparing my meals in the kitchen. My bedroom is a few steps from the sitting room and the kitchen. My entire house is tiled. The bedroom has a bed, wardrobe, table, and chair only. I have a combined bathroom and toilet; the bathroom has a shower equipped with towels, bathing soap, and toothbrush. These places tend to pose risks to my safety. First, having the whole house tiled most of the time, I sometimes slide and fall. Secondly, the house is big, and living alone often makes me feel insecure because I can be attacked easily. Thirdly, in most cases, I usually forget cooking meals with the gas cooker and only to remember when I smell burnt food. The changes which I would need to be made in my home to improve my safety are first, the tiles to be removed from the bathroom, which becomes more slippery, especially when I am bathing and for the rest of the house floor mat to be placed on each room. Secondly, I need a helper in cooking and doing other house chores. These changes would improve my safety because it will reduce the number of times I usually fall due to slipperiness and feel secure when I have a helper.

Complaints About Dietary Services Contributed by Cindy K. Manjounes, MHA, EdD, Lindenwood University. Complaints from residents have started to surface regarding availability and selection of items on the menu. Residents on restricted diets as well as vegetarians have limited choices. The facility offers a nonselective menu, but, to the extent possible, accommodations are made. Recently, meal hours have been reduced because of staff shortages in the dietary department. The administrator was invited to this month’s resident council meeting in which some residents became emotional, and the council’s president stated, “We used to look forward to meal times, but things have gone downhill. The hours have been cut and we are rushed through the meals. Some of us are thinking about moving to another facility even though we like the nurses here. They are really caring. We have heard that the home has a new food service manager. This person needs to straighten things out before we all leave.” Attendance in the dining room has dropped because residents have started to request in-room meal delivery so they can take the time to eat their meals. Some CNAs have been grumbling about the extra work they have to do to deliver meals and pick up afterwards. Questions 1. Identify the main issues that need to be addressed. What may have caused these problems? 2. Prioritize the issues based on their potential impact on the residents, staff, and the facility. ( This part of the question requires impact of issues on each: resident, staff and facility) 3.How should the administrator go about addressing the issues

Case: You are providing care for an 83-year-old woman named Lucy. Lucy was born in Lithuania, but moved to Australia in her early twenties. She misses her home country a lot, and often talks about her experiences there as a young woman. Lucy says that she enjoyed living in Australia with her husband, but that since he died a year ago, she has been feeling more and more lonely. Her children do not live close by, and she does not see them very often. She would like to participate in social and community activities but does not know where to begin. Lucy used to enjoy singing and was once in a choir. However, she gave that up when her husband got sick, and hasn’t sung in a few years. You raised the idea of joining a choir again, but she said that her voice is probably too weak, and she doesn’t feel confident singing in front of others. (PE 1) Identify two or three services or opportunities that you could discuss with Lucy, that might empower her to feel more independent and happy. Without imposing your own values or attitudes, how could you support Lucy to better express her identity and preferences when discussing possibilities for services? Name one thing you could do that will show respect for Lucy’s social and cultural background when discussing potential opportunities and services with her. Select one of the services or opportunities that you identified in Question 1, and explain how you could provide it to Lucy, in a manner that respects her preferences and upholds her rights. What organisational policies and procedures, or duty of care requirements, might need to be adhered to when providing this service for Lucy? Explain how you would take these into account. What other information and/or support could you provide Lucy to encourage her to engage as actively as possible in this service? You are discussing service possibilities with Lucy, including the fees that some of them may incur. Lucy mentions to you that she does not have her credit card on her, because her son borrowed it from her around six months ago to pay off a debt. She tells you that she has asked him about the card once or twice, but he hasn’t brought it back and she hasn’t wanted to push it. She is not sure if he is still using it, and has not checked her account balance. Lucy assures you that she is fine using her debit card, and that she is sure her son has just forgotten to return the credit card to her. Do you think Lucy may be experiencing financial abuse? Explain your answer, giving examples of signs that she may be being abused, or having her rights breached. Explain how you would respond to this situation. Outline how you would maintain Lucy’s privacy and confidentiality when reporting and managing this situation. In your answer, take into account the role of organisation policies and protocols.
