-In 1997, two drugs, bromfenac and troglitazone, prompted the Food and Drug Administration (FDA) to begin a cycle of yearly conferences to review issues of drug – induced liver injury (DILI). This was after the two drugs had shown serious fatal liver toxicity. Drug application reviewers were the first invitees to the conference in 1999. Academic consultants and pharmaceutical industry were later involved in 2001. As a result, FDA released a guidance entitled: “Guidance for Industry Drug-Induced Liver Injury: Premarketing Clinical Evaluation” (Senior 2014). This presentation was a rational approach was meant to assist in assessment of liver safety in clinical trials.
FDA identified DILI as a key area of focus in a joint effort to broaden the industry’s knowledge in liver toxicity. Since the guideline in 2009, DILI has been the main cause of most drug marketing withdrawals. FDA proposes standard non-clinical toxicity as the cornerstone for preventing hepatotoxicity in humans despite their predictive power being unsatisfactory (Senior 2014). As such, new tests with improved sensitivity and specificity are needed are required to predict hepatotoxicity in clinics.
The guidance describes observations needed to evaluate indicators of hepatotoxic potential. According to Senior (2014), these observations include detection, reaffirmation and examining of liver test abnormalities. The industry then does close evaluation excluding other possible caused of liver toxicity. Lastly, the FDA guidance requires the industry to those affected. This largely involves supportive care till return to baseline status or normality. FDA also makes specific recommendations in the presentation on why some people are more susceptible to DILI. It proposes the use and evaluation of Hy’s Law cases that are discovered through clinical examination.

-Nsaids are a commonly used drug that can be bought over the counter without a prescription. NSAIDS are used for patients that have chronic pain, stroke, atherosclerosis and postoperatively this medication is used very commonly. Like all medications Nsaids have risks and benefits. One major risk is bleeding. It is important for patients who are going for surgery to stop this medication the morning of surgery to prevent the risk of bleeding during surgery. NSAIDS are also contraindicated when taking lithium. NSAIDs are known to increase the effects of lithium can can lead to lithium toxicity when taken together. NSAIDs when combined with certain medications can be very dangerous. Nsaids are commonly taken by most people not only geriatrics but also younger population take nsaids for migraine and for menstrual cramps.
Some benefits of Nsaids is that it is easily accessible OYTC and that is very affordable for most people. NSAIDS are very effective for mild to moderate pain. Possible negative side effects include liver and kidney damage, nausea, vomiting and neuropathy. NSAIDs have also been used in combination with chemotherapy drugs in treatment of some cancer. This is now being dispelled and scientists are finding that NSAIDs don’t have any ant cancer causing properties and have now been contraindicated for patients on chemotherapy, the benefits don’t outweigh the cons of taking this drug. Stronger opioids are preferred when treating cancer pain. One benefit of taking an NSAID is that is is not habit forming and there is very low risk of becoming addicted to this medication in comparison to opioids/ narcotics which are extremely addictive and have higher risk of overdose.

-FDA approach for liver safety for new drugs
The food and drug administration initiated a sequence of annual meetings to deal with issues of drug induced liver injury. This was due to approval of two drugs troglitazone and bromfenac that led to significant lethal liver toxicity. In the meetings reviewers of new drugs are invited to the meetings and the meetings are opened to pharmaceutical industry, academic consultants to industry, and food and drug administration (Senior, 2014).. The slides shown in the meetings are posted on the internet and availed to the American association for the study of the liver diseases. This has raised awareness on new drugs that result to liver injury.
The food and drug administration also developed a program called eDISH meaning evaluation of Drug Induced Serious Hepatotoxicity which is useful in examining drug induced liver injury in clinical trial subjects. This has been useful as the food and drug administration has been able to curb hepatotoxicity (Senior, 2014). The food and drug administration however is yet to find a suitable and best way of estimating the probability that a drug in question actually caused the given liver injury. The food and drug administration also carries a yearly educational conference on drug induced liver injury for the food and drug administration reviewers.

-The management of acute and chronic pain often includes opioid therapy. In both the acute and chronic pain settings, however, opioids have several disadvantages including risk of nausea and vomiting, somnolence, constipation, respiratory depression, androgen deficiency, physical dependence, and tolerance. Opioid medications also carry a risk of abuse or addiction by either the patient or non-medical users. For these reasons, consideration of non-opioid strategies for pain management is beneficial. While opioids will certainly continue to have a place in pain management despite their disadvantages, the use of non-opioid medication options may limit the amount of opioid necessary or even result in improved pain control. In fact, given that the majority of both acute and chronic pain is thought to be complex and multi factorial, a multi modal analgesic approach is ideal for management. Acetaminophen is typically considered first-line therapy for chronic pain conditions, including osteoarthritis (OA) and chronic pain associated with total knee arthroplasty. However, many times patients are treated inappropriately with opioids instead.
Acetaminophen is considered first-line treatment for many pain conditions because of its safety advantages. Compared with other analgesics available, acetaminophen is associated with very few adverse events and is considered safe to use chronically. Healthcare providers strive to “first do no harm. Therefore, for most chronic pain conditions, a trial of acetaminophen should be used and monitored for effect before initiating other analgesics, including opioids. However, acetaminophen is not without risks. Safety concerns associated with acetaminophen include serious liver damage if ingestion is greater than the recommended total daily dose of 4,000 mg. Long-term high doses of acetaminophen and accidental overdose from inadvertently consuming multiple acetaminophen products are the leading causes of acute liver failure in the U. S. For this reason it is important that the patients who are utilizing acetaminophen chronically for pain are educated on proper dosing and administration, and are aware of any other acetaminophen-containing products they may use. The disadvantage of acetaminophen is that it often cannot provide a sufficient analgesic effect and does not exert a specific anti-inflammatory effect, causing many patients to discontinue or switch therapies.
