Nursing research falls within paradigms which are positivist and naturalistic. Both paradigms oppose assumptions about views and reality of the world. Example of reality, the positivist believes that a single reality exists and that can be measured, while naturalistic paradigm are multiple realities that are continually changing and this makes it very difficult to measure. The two main types of research methods are quantitative and qualitative.

Quantitative research aligns with the positivist paradigm, while qualitative research aligns with the naturalistic paradigm. Quantitative research is a formal, objective, deductive approach to problem solving. While qualitative research is more informal, subjective, inductive approach to problem solving.

       Both methods are appropriate for conducting research; Selection of method to use depends primarily on the research questions being asked. These questions move from the research problem and purpose statement. For example, testing a new fall prevention program within hospital would require to obtain a baseline fall rate before the program and then again after full implementation of the program. Statistically, one could compare rate of falls before the new program with the rate of falls after the new program.  Unit of analysis would be numbers and would lend itself to a quantitative design. However, if interested in studying the impact of falls on patient’s quality of life, obtain that information through a personal interview. The unit of analysis would be words, and a qualitative method would be the most appropriate approach to analyze this data (Houser, 2008).

Using both methods can be advantageous to researchers because both methods can be used to conduct studies. Qualitative studies use word to develops theory and frameworks and quantitative studies use numbers to test theory. It is also advantageous to use both studies when one study is conducted , the researcher can continue to validate the study after it is found to be reliable (Biddix, 2018).

Disadvantages of both studies comes into play in relations to bias and analyzing data. Bias takes away from the truthfulness of the study because the researcher will use the findings to their advantage.  Elimination of bias in a quantitative study is “achieved when validity and reliability are established and in qualitative study it is achieved “when reliability, applicability and consistency are established” (Biddix, 2018). 

Qualitative researchers use print media and electronic media to analyze data.  The disadvantage of data analysis is getting the necessary information for the study when the information from these sources are very large (Biddix, 2018).

Windshield surveys are useful when the area you want to observe is large and the aspects you are interested in can be seen from the road.  A WALKING survey might be utilized when you’re seeking to understand things that are hard to see from a moving vehicle.

Pick an area that is manageable.  If you live and work in a large city, you’re probably only concerned with a small section, or a particular segment of that city.  For example, if you lived in New York City, you might select just Manhattan or the borough of Queens.  Your choice will help shape your understanding of the community, so you should make it thoughtfully.  Which areas will best reflect the aspects of the community that you want to know about?  Which locations best relate to the work you want to do?

Consider how the time of day, the day of the week, and the season might impact your survey.  You may want to repeat your survey more than once in order to capture the differences between community conditions or activities at different times.

General Tips:

· Use a map.  Google maps on your smart phone or similar services are excellent resources

· Enlist a spouse, friend or family member to assist you so that you can make notes and record your observations while someone else drives.

· Drive major and minor streets, particularly in residential neighborhoods.

· Pull over at regular intervals to make notes.

· Try to remain inconspicuous but take pictures to refresh your memory about buildings or areas of interest or concern.

· To truly experience the community, take part in everyday activities.  Take public transportation, eat in a local restaurant, buy something in a drugstore or grocery store.  This will give you a chance to listen to people’s conversations and to get a sense of how they interact.

· Go inside public buildings and cultural institutions

· Sit down in a quiet place to observe the population and to take notes.

· Consider the community’s outstanding assets.

· Consider the community’s biggest challenges.

· Explore the unexpected.

· Note the aesthetic quality of the community, either positively or negatively.

One of the historical experiments that was instrumental in shaping the future of ethics and regulations in research is the famously known Tuskegee experiment which was conducted without the participants giving their informed consent to participate in the study and the researchers did not inform the participants the risks and benefits of participation and the reason behind conducting the research. The Tuskegee Institute together with the Public Health Service started the study in 1932 with the aim of finding out the history of syphilis among the black men hoping that they would start treatment programs. 600 men participated in the study. 399 of the men had syphilis while 201 men did not have syphilis. The researchers were lying to the men that they were treating them for having bad blood which was a term that was used to refer to long term illnesses associated with fatigue, anemia and syphilis. This study was supposed to run for 6 months but it continued up to 40 years (Center for Disease Control, 2017).

The participants were compensated with free medical examinations, meals and burial insurance. What went wrong with the study was discovered 40 years later after a press story lead to public outcry and a review of the study was done by a panel of 9 members. The panel found out that there was no evidence to show that the men had been informed about the study and its main purpose and no facts were provided to them at all. They agreed to be examined and treated freely. Still, in 1947, after penicillin was found to be a cure for syphilis, the men were never treated using penicillin neither were they given the chance to quit from participating in the study. The study ended in 1972 and in 1973 a lawsuit was filed on behalf of the involved participants and the government promised that it would give lifetime medical benefits to all those who survived, their wives and offspring’s and they would also offer burial services to them. On 16th May 1997, the Presidential apology was given by President Clinton on behalf of the nation and in 2001 the Presidential Council on Bioethics was established. The last person to receive these benefits was a widow who died on 27th January 2009 (“Tuskegee Syphilis Study and Its Implications for the 21st Century,” 2015).

Gallagher (2014) suggests that to make the study ethical and valid, the participants were supposed to be informed about the objective behind their required participation in the study and in case they never wanted to participate, they should not be coerced into the study. Secondly, the participants were supposed to be treated using penicillin which was discovered to cure syphilis in 1947 and this would have reduced the deaths. More so, there were no records on the number of deaths that resulted from the study. The researchers were supposed to have followed up on the participants and keep records whether they died from syphilis. In a nutshell, the study violated the rights of the participants by keeping them on the dark on what was happening.
