One critical component of Evidence Based Practice is to ensure that practice change is part of an organization’s culture so it will continue to impact outcomes over time. There are barriers that may prevent my Evidence Based Practice change proposal from continuing to obtain the same desired results six months to a year from now. My PICOT statement is, In adults with tobacco dependency (P), would providing teaching/research about tobacco cessation (I) compared to medications to assist in tobacco cessation (C) lead to a decrease in tobacco dependency and better health outcomes (O), after four weeks of intervention/quit date (T)? One barrier is new staff coming on board and not knowing the proper way to educate patients on tobacco cessation. The current staff will receive proper education on how to educate tobacco dependent patients. To assist new staff with the process change, we can create required computer based learning assignments (CBL’s) that have to be completed before coming out of orientation. This will allow all new nurses to be up-to-date on the education process. Another barrier is language. Not only is language a barrier in education, but other forms of communication as well. Upon communicating with a patient who does not speak English, the health care team must first decide what language the patient understands best, and in what way the patient learns best. Once the assessment is completed, the nurse can obtain an interpreter. At our facility, there are interpreters on wheels; this is an IPad that we bring to the patient and an interpreter comes on live to speak to the patient in their specific language. Not only can we use the interpreter, but provide written education in different languages as well. Throughout the education process, it is important to remember communication techniques. A few communication techniques are listening, asking open ended questions, and having an open posture (Elsheikh, 2018). Change initially can be a good thing and take off quickly. Remote care and diagnosis is a continuing and increasingly important method for nurses to help deliver care to patients to promote safety and enhance health outcomes. Understanding best EBPs and building competence in delivering nursing care to remote patients is a key competency for all nurses. Additionally, in some scenarios, while you may be delivering care in person you may be collaborating with a physician or other team members who are remote. Understanding the benefits and challenges of interdisciplinary collaboration is vital to developing effective communication strategies when coordinating care. So, being proficient at communicating and working with remote health care team members is also critical to delivering quality, evidence-base care.

After surgery he was asymptomatic.

No delusional or paranoid thought processes noted. Judgment and insight appear to be age-appropriate. He is not endorsing active suicidal ideation, but does admit that he often thinks about himself being dead and what it would be like to be dead.

Then she ask the pt to change dress. Then inserte the IV with the fluid. Bed in lower position and call light with pt.  Also take three consent from pt one for procedure one of anthesea and one for blood. Also ask pt to remove the jowlary and glass and dentior. And check the lab result if any abnormality and inform thr surgon and anethesea. Then give hand off report to OR nurse . note in the perioperation no time out only for some surgery.

The “Pulse” Check.  Where are you in your journey and how are you doing?

DQ2 of Week 5 is an opportunity for you to self-assess and reflect on your journey. You can write about it expressing your navigation through the Role course. Model from the scripts in this week but be specific to your experience.

Evaluate how you have achieved course competencies and your plans to develop further in these areas. The course competencies for this course are as follows:

1. Explore the historical evolution of the advance practice nurse.

2. Differentiate the roles and scope of practice for nurses working in advanced clinical, education, administration, informatics, research, and health policy arenas.

3. Analyze attributes of the practice arena such as access and availability, degree of consumer choice, competition, and financing that impact advanced practice nurses and their ability to effectively collaborate with other health professionals.

4. Integrate evidence from research and theory into discussions of practice competencies, health promotion and disease prevention strategies, quality improvement, and safety standards.

5. Identify collaborative, organizational, communication, and leadership skills in working with other professionals in healthcare facilities and/or academic institutions.

6. Synthesize knowledge from values theory, ethics, and legal/regulatory statutes in the development of a personal philosophy for a career as an advanced practice nurse.

Guidelines: Support your responses with scholarly academic references using APA style format. Assigned course readings and online library resources are preferred. Weekly lecture notes are designed as overviews to the topic for the respective week and should not serve as a citation or reference.

In your discussion question response, provide a substantive response that illustrates a well-reasoned and thoughtful response; is factually correct with relevant scholarly citations, references, and examples that demonstrate a clear connection to the readings.
