 In terminally ill patients and their families (P), how does the provision of educational programs to demystify benefits of hospice care (I) as compared to informal community education (C) influence their ability to increased awareness of the best options and benefits of hospice care (O) within a period of hospital stay (T)? 

Justification for the Clinical Problem

 This paper describes a hospice educational program for terminally ill patients. The main focus of this study is the lack of accurate and inconsistent information on the benefits and options of hospice care. There is a large gap of eligibility, philosophies and knowledge concerning hospice care for patients facing end of life transition. Most studies that have been carried out on the topic have either been biased or provided less detailed information on the importance of creating awareness of the benefits of hospice care for terminally ill patients. Lack of knowledge about the importance of hospice care has led to increased number of people living their last days without receiving the benefits of this service (Garcia, 2018). 

 Most families learn about the benefits of hospice care care when it is too late for the patient. In addition, the families learn about the advantages of the service once the patient has already been admitted in the program. The main reason for enrolling their terminally ill patients to the program is not because they know its benefits but for the lack of another option. The significance of this study is to help patients and families to enroll in hospice care early enough in the course of a life limiting illness for them to receive comfort care and experience easier transition. 

 According to Tofthagen, Guastella, & Latchman, 2019), the need for palliative and hospice care program has increased as people are living with complex diseases with no cure and population ages. The study suggests more hospice care professionals will be needed in the near future to meet the increasing demand for the services (Tofthagen, Guastella, & Latchman, 2019). Lack of knowledge on the importance of these services has resulted in families keeping terminally ill patients at their homes. The authors of this study suggest that providing educational programs to communities could help reduce the number of terminally ill patients who are not receiving these services (Tofthagen, Guastella, & Latchman, 2019). 

 Another study conducted in 2020, found out that healthcare professionals in the hospice care setting can only provide effective services to patients if there is a conceptual framework to guide practice and education (Dyess et al., 2020). The study suggests that most people are unable to access hospice care because of lack of knowledge. “The framework builds on theoretical caring to convey elements of relational, holistic and compassion; articulates inter-professional tenets for guiding values; and aligns with constructs for palliative and hospice best practices” (Dyess et al., 2020). The study suggests that such a framework invites leaders as well as clinicians to develop educational programs to promote awareness about hospice services. 

 According to Allo et al., (2016), healthcare professionals do not have exposure to provide quality home based care. This is as a result of lack of enough education among healthcare practitioners to attend to home-based patients. The study suggests that participation in patient home visits is an effective way to provide education in communities about the benefits of hospice care (Allo et al., 2016). This approach does not cause any form of distress to both patients and their families. As a matter of fact, families who participated in this study were quick to enroll their patients to hospice care (Allo et al., 2016).

 In another study conducted in 2019, the authors argue that most terminally ill patients do not have access to hospice services due to lack of knowledge in the communities (Livingstone, Welstand & Ryan, 2019). However, such patients would enroll to hospice care services if they were aware of its benefits. Livingstone, Welstand & Ryan, (2019), found out that the fears associated with end of life include not being able to stay at home, burden to the family and also fear of undignified death. Most terminally ill patients would therefore enroll to such programs to reduce these fears if they had enough knowledge. 

Article that Best Supports the Topic

 In a study conducted in 2018, the authors argue that hospice care is a service that has been widely misunderstood and underutilized (Garcia, 2018). Communities are not aware of hospice services. This results in increased suffering for patients having life limiting illnesses. Misconceptions by communities also lead to the lack of enrolling terminally ill patients to the service. Most people believe that the service is only provided for six months. People also don’t understand their illnesses as well as the treatment goals for hospice care. While there are many hospice care organization in communities, lack of clarity on who should handle this conversation exacerbates the situation (Garcia, 2018). 

 Limited knowledge, misconceptions and poor communication among patients and within the health care profession, have led to terminally ill patients not being referred on a timely manner (Garcia, 2018). Communities do not have enough knowledge to identify the identify patients who need hospice care. Late referrals have led to patients being denied access to hospice care at the end of their life. The underutilization of these services has resulted in poor communication and ineffective identification of those patients who deserve end of life care. The underutilization of hospice care leads to poor quality of life for terminally ill patients and also increased healthcare services (Garcia, 2018). 

  This article provides ways in which terminally ill patients and their families can access education programs to help them understand the benefits of hospice care services. The article also provides insights on the importance of providing educational programs to communities (Garcia, 2018). The other article focus more on the importance of providing hospice care rather than the importance of educational programs on hospice care services. Unlike the other selected articles, this article provides recommendation educational programs on the importance of hospice care can be administered in the community. By implementing this recommendation most people suffering from life limiting illnesses can access hospice car and end their life with dignity.

Vaccine Controversies

There is no doubt that the topic of vaccine has received numerous fears because of its below 100% effectiveness, among other reasons. On the other hand, the campaign for vaccine has been strongly recommended because of the lives saved through it and the diseases it has prevented such as polio and small pox. In this discussion, I will attempt to look at the two perspectives with a view of shading light to what is the most important, severe and useful to mankind regarding vaccine controversies in the world today.

Since vaccine was introduced, there have been rivalry by religion on unfounded facts. Owing in mind many unvaccinated children have passed away, religions remain firm with their Simon says hence nations give in to their negative ideas. On the question of safety versus sanity, the cell has some viral vaccines which has to be gotten from humans. Rubella vaccine is derived from tissues taken therapeutic abortions performed in 1960s.

In Kerala a magazine had an article of certain minor post-vaccine adverse events causing death. A boycott of vaccine was called and vaccines called “agents of death”. The respond to that was predictable as there was a heated debate on television channels. Later the article was removed since there was opposition lead by Indian Medical Association but by then many had already accepted the vaccines should be eradicated. Before this, in Malappuram district of India, religion groups said that vaccines was targeted to demolish a certain religion. As a result 100% of children in that district have been vaccinated but only 74% are fully protected. A campaign is needed being led by IMA (Indian Medical Association) to stop the spread of myths against vaccines (Vasudevan, 2016). 

In conclusion, vaccines save lives but the need to draw a line between what is right or acceptable and what are wrong calls for a lot of compromise from all parties. The examples above show that more research also need to be undertaken to find better ways to cope with allergies from vaccines so as to improve on survival rate after vaccination.

Communication is a process through which information, understanding, and perceptions are passed from one person to another; communication is very important in nursing as it is an integral part in the field. Many of the times when we meet for the council meeting, staffs have been coming up with ideas on how patient care can be improved and the process has been encouraged as it seeks to come up with new ways of improving service to the patients. In some instances, there are ideas which I approve without involving the management, and there are others which need the approval of the top management to be implemented.  

My unit council had noted that patients have been walking for long to access services within the facility, this is because the parking lot is located fur and sometimes the patients have been facing challenges (Huber, 2017).To present the changes to the upper management I used oral presentation though there were some visual tools to help in illustrating issues which cannot be presented verbally. During the presentation, I made sure to explain how the idea come about and how my unit has come up with a solution to the problem. In the discussion, we talked of the possible outcome if the idea was implemented and how it would improve service to the patients. The issue could be solved by coming up with valet parking for emergency patients. By the time we left the boardroom, the management had approved the implementation of valet parking for emergency patients. One issue with the Patient Protection and Affordable Care Act (also known as Obamacare) is cost control. By failing to maintain cost control, Obamacare may not be able to ensure widespread access to healthcare (ACEP, n.d.). The main issue with cost control is distributive justice (ACEP, n.d.). Patient responsibility is another issue found within Obamacare. There are fewer taxes on harmful behaviors and more positive incentives (ACEP, n.d.). The issue with this is they are silent on harmful behaviors that cause the highest amount for the healthcare system. The last issue found in this article is about crowded emergency departments. One study found that people were visiting the emergency department even though they had a primary care physician (ACEP, n.d.). I agree with all of the issues presented. I know personally how hard it is to try to get in to see my primary care physician on short notice. It is frustrating when a sudden illness happens, and I can’t get in to see my doctor for three weeks. This makes me have to choose to either go to the ER or wait it out and be miserable and possibly get worse.

A major issue that came up was the growing population. I think that while attempting healthcare reforms in the future, this should be taken into account. Not only should they study the population growth, but attempt to estimate whether it will continue to increase or if it will decrease, and include that information in the planning of any reforms.

The article described first what the morals or fundamental goals of every American want and expect from our health care system. There are four fundamental goals and desires that have shaped the US health care system.  First, there’s quality care.  Second, there’s freedom of choice in health care.  Third, health care must be affordable. Fourth, we want our fellow citizens to share in the considerable benefits of health care (ACEP, n. d.).  Some of the ethical issues the article referred to were for instance a serious constitutional challenge to the PPACA is grounded on claims for individual liberty: it asserts that requiring Americans to purchase health insurance exceeds the authority placed in the hands of Congress by the US Constitution. Other countries, however, accept significant responsibility for the material welfare of their fellow citizens. These nations clearly place a high priority on the egalitarian goal of universal access to health care (ACEP, n. d.). I grew up in Germany where the employer and employee both pay 50% of the healthcare insurance and everyone has to have insurance. Another ethical issue the article was dealing with was tort reform of cost containment. I can see where defensive medicine results in costly diagnostic testing, hospitalizations, and therapeutic interventions. There needs to be some legal intervention to prevent frivolous lawsuits (ACEP, n. d.).  Access of care and emergency room crowding was another ethical and moral issue discussed. Many Americans who have been hard to insure because of pre-existing conditions had no choice but to go to the emergency room. I know I have a hard time to get in with my primary care physician and I have been told that I can always be seen at the ER if it gets too bad. I don’t agree with this because it’s passing the bug mentality. The last ethical issue the article talked about was end-of-life issues. I agree that communication, education, and joint decision-making plays a vital role in this situation.

I think for future healthcare reform we really need to look at how insurance companies can turn away customers with pre-existing health conditions. 

From a healthcare administrator’s perspective, the ethical issues in regards to the Affordable Care Act, present unique issues that still need to be addressed. The main issues presented in the article by the ACEP (n.d.) are, ”cost containment, public health, access to care, ED crowding, and end-of-life issues.” I would definitely have to agree that these issues are the leading cause of the fiscal fiasco that is the American healthcare system. According to the OECD (2018), on average, the US spends twice ($10,348) what other comparable western countries ($5,169) spend on health care. This ties into the ethical ramification of distributive justice in a fiscal perspective as we as a nation are spending so much money for a product that is not comparable any better than other western countries. The cost of access is so high that it discourages those who can’t afford it from receiving proper medical care. Cost transparency for multiple payers and standard rules for submission was suggested by Blanchfield (2010) that would, hypothetically, save $7 billion in billable time. Saving both time and money in this instance would effectively increase access to care as well as ED overcrowding. Utilizing savings from wasted man-hours increases funds available for ED renovations.
