Initial Post Instructions
The study of ethics and philosophy is one that brings many different kinds of “thinkers” together. One person’s philosophy on ethics is another person’s philosophy on evil. We will be working this term on constructing personal ethical bases and understanding how ethical codes (both personal and professional) are created and followed.

To start us thinking about the different areas of philosophy and ethics, and how we fit into the different molds or world views, let’s imagine the following scenario:

It is 2019. The federal law banning female circumcision is still under appeal in the courts. You are a nurse assisting a plastic surgeon at a local hospital. The plastic surgeon comes from a country where they practice “female circumcision”. This practice is also sometimes called “female genital mutilation”.

You are not a member of the doctor’s culture, but reside in a state where this practice is still legal. The plastic surgeon has agreed to perform this practice on a young girl, the daughter of a friend of the surgeon. The friend has authorized the procedure. The girl only knows this is a custom. You did not know that today you would be asked to assist in this procedure. You can refuse to participate (your job may be on the line in the future due to that decision). Or, you can assist the surgeon. What ought you to do? We now want to examine the ethical issues involved. To do this, let’s look at the role of relativism, moral truths, and other issues.

Initial Post Instructions
For the initial post, address the following questions:

• What would a subjective moral relativist say about what this doctor is doing? Do you agree with the subjective moral relativist? Why or why not?
• Examine what a cultural moral relativist would say here. Do you agree with the cultural relativist? Why or why not?
• Name and evaluate general criticisms of cultural relativism as being the wrong moral approach.
• Is there an objective moral truth about any of the possible actions by the nurse and/or doctor in this case? Why or why not?

Think About: OUR NATIONAL BULLYING EPIDEMIC

No matter what your age or where you are from, you almost certainly experienced elementary school, middle school or high school bullying at some point in your childhood (either as a victim, a witness, or maybe even the perpetrator). Keep in mind that many of us actually fit into all three of these categories. Bullies have typically been bullied and bullying victims often go on to bully others. 

First Response Post: Whatever your perspective or place in the bullying aggression, try to remember how the school addressed the behavior. Answer the following questions: 

What was the response of the school administration (teachers, principal, counselors, etc)?  Was the response sufficient (did it stop the behavior)? If the response was not sufficient, what more could/should  the school administration have done?

  Discussion 05.1: Muddiest Point

 At the beginning of this lesson write a short one or two paragraph posting entitled “The Muddiest Point.” In these few sentences write down the most unclear topic or idea covered in the last lesson or in your instructional materials. It is to be used by your instructor to assess areas where instruction was weak and where more time needs to be spent for your comprehension. 

Pulmonary Function:
D.R. is a 27-year-old man, who presents to the nurse practitioner at the Family Care Clinic complaining of increasing SOB, wheezing, fatigue, cough, stuffy nose, watery eyes, and postnasal drainage—all of which began four days ago. Three days ago, he began monitoring his peak flow rates several times a day. His peak flow rates have ranged from 65-70% of his regular baseline with nighttime symptoms for 3 nights on the last week and often have been at the lower limit of that range in the morning. Three days ago, he also began to self-treat with frequent albuterol nebulizer therapy. He reports that usually his albuterol inhaler provides him with relief from his asthma symptoms, but this is no longer enough treatment for this asthmatic episode.

Case Study Questions

1. According to the case study information, how would you classify the severity of D.R. asthma attack?
2. Name the most common triggers for asthma in any given patients and specify in your answer which ones you consider applied to D.R. on the case study.
3. Based on your knowledge and your research, please explain the factors that might be the etiology of D.R. being an asthmatic patient.

Fluid, Electrolyte and Acid-Base Homeostasis:
Ms. Brown is a 70-year-old woman with type 2 diabetes mellitus who has been too ill to get out of bed for 2 days. She has had a severe cough and has been unable to eat or drink during this time. On admission, her laboratory values show the following:

• Serum glucose 412 mg/dL
• Serum sodium (Na+) 156 mEq/L
• Serum potassium (K+) 5.6 mEq/L
• Serum chloride (Cl–) 115 mEq/L
• Arterial blood gases (ABGs): pH 7.30; PaCO2 32 mmHg; PaO2 70 mmHg; HCO3– 20 mEq/L

Case Study Questions

1. Based on Ms. Brown admission’s laboratory values, could you determine what type of water and electrolyte imbalance does she has?
2. Describe the signs and symptoms to the different types of water imbalance and described clinical manifestation she might exhibit with the potassium level she has.
3. In the specific case presented which would be the most appropriate treatment for Ms. Brown and why?
4. What the ABGs from Ms. Brown indicate regarding her acid-base imbalance?
5. Based on your readings and your research define and describe Anion Gaps and its clinical significance.

Submission Instructions:

• You must complete both case studies
• Your initial post should be at least 500 words per case study, formatted and cited in current APA style with support from at least 2 academic sources

Week 1 Discussion Forum

Compare and contrast the actions associated with leading, managing, and following, giving examples of how each applies to the enhancement of the work team. Do you feel these are fixed positions or are there times when the nurse could move from one position to another (give example)? 

Be sure to apply concepts from the readings and research. You must cite (with reference) at least one source: your textbook, scholarly resources, or ATI textbook (no blogs, Wiki, or other school of nursing website) in current APA Style.
