Apex Managed Care

You are an auditor assigned to evaluate Apex Managed Care Inc.’s Open MRI health plan according to HEDIS performance measures. As part of your evaluation, you must fully explain what the performance guideline actually measures and what information you would look at to determine whether the Open MRI is complying with the standard. Be sure that you apply each of the eight (8) dimensions/guidelines specifically to an Open MRI clinic. Cite all sources in a bibliography formatted in APA style. 

For this project, you will identify a case study and then develop a nutritional care plan for the client associated with that case.  Please read this entire document to gain understanding of the project scope and those tasks you will complete.  

Task 1:

a.  Choose from one of the following stages: pregnancy, infancy and childhood, adolescence, adulthood, and old age.

b.  List the specific nutrition issues affecting individuals in your selected stage.

Task 2:

a.  Choose a disease or medical condition that could possibly, or commonly, affect your individual.  This could be anything ranging from diabetes to cirrhosis.

b.  Using the ABCDs of nutritional assessment, create a case study based on an actual client or patient.  You can also make one up.  Include the following in your case study:

(i).  Anthropometrics, relevant biochemical tests, clinical assessment, and dietary intake analysis.

c.  Be sure to include a list of common medications that may be used to treat your patient’s condition and identify potential herb/nutrient/drug interactions that may be relevant.

d.  Identify your client’s cultural background and give clues as to their socioeconomic status and psychosocial variables.  For example, your

patient may be a senior living alone on a fixed income or might be living in

a nursing facility with reputed staff.

Task 3:

a. Create a nutritional care plan for your client. This will include an

evaluation of nutritional risk, a list of interventions, and a list of expected

outcomes.

b. List the goals of medical nutrition therapy for your patient and suggest an

appropriate diet. Does your patient/client require a dietary modification of

their regular diet? Are enteral feedings by tube necessary? If so, what

formula will you use and why? Describe your recommended method of

administration.

Task 4:

a. Describe your nutrition education teaching session with your patient

and/or their family. What teaching methods will you use (explanation,

discussion, demonstration, handouts, etc.)? In your own words, write a

paragraph detailing three specific points that you will need to teach your

patient about his/her new diet. In addition, give at least one tip to avoid

potential herb/nutrient/drug interactions.

The topic this week asks you to apply what you have learned to the following case study.

As the school nurse working in a college health clinic, you see many opportunities to promote health. Maria is a 40-year-old Hispanic who is in her second year of nursing school. She complains of a 14-pound weight gain since starting school and is afraid of what this will do to both her appearance and health if the trend continues. After conducting her history, you learn that she is an excellent cook and she and her family love to eat foods that reflect their Hispanic heritage. She is married with two school-age children. She attends class a total of 15 hours per week, plus she must be present for 12 hours of labs and clinical. She maintains the household essentially by herself and does all the shopping, cooking, cleaning, and chauffeuring of the children. She states that she is lucky to get 6 hours of sleep per night, but that is okay with her. She lives 1 hour from campus and commutes each day. Using Healthy People 2020 and your text as a guide, answer the following questions.

1. What additional information would you like to gather from Maria?
2. What are Maria’s real and potential health risks?
3. Why is Maria’s culture important when obtaining the health assessment?
4. Pick one of Maria’s health risks. What would be one reasonable short-term goal for this risk?
5. What nursing interventions would you incorporate into Maria’s plan of care to assist her with meeting your chosen goal? Please provide rationale for your selections.

 Hematological System

Often in the medical field, an emphasis is placed on the importance of the heart and the lungs. While these two organs are vital to the sustainability of life, the blood is what makes the cardiovascular and respiratory systems function—it’s the connection for the heart and lungs. For this reason, disorders of the hematological system can be potentially devastating for patients. Consider the case of Connie Prochnow. Connie was diagnosed with leukemia after seeking medical care for bruising, shortness of breath, and exhaustion. Her blood disorder resulted in alterations that impacted other body systems, including her respiratory system (UW Health, 2012). Since the heart and the lungs rely so heavily on the blood, it is important that hematological disorders are quickly identified and managed.

• Chapter 19, “Structure and Function of the Hematologic System”

 

This chapter examines components of the hematologic system, development of blood cells, mechanisms of hemostasis, and hematologic value changes in pediatrics and geriatrics. It also focuses on common blood tests for hematologic disorders.

 

• Chapter 20, “Alterations of Hematologic Function”

 

This chapter focuses on common alterations of hematologic function, including alterations of erythrocyte function, leukocyte function, lymphoid function, splenic function, platelets, and coagulation.

 

• Chapter 21, “Alterations of Hematologic Function in Children”

 

This chapter expands on alterations of hematologic function by presenting disorders that affect children, such as disorders of erythrocytes, coagulation, and platelets.

This chapter begins by exploring the anatomy and physiology of blood and the coagulation system. It then examines two types of anemia caused by red cell disorders. White blood cell disorders and platelet disorders are also examined.

Discussion: Anemia

In clinical settings, advanced practice nurses often encounter patients with blood disorders such as anemia. Consider the case of a 17-year-old girl who is rushed to the emergency room after suddenly fainting. The girl’s mother reports that her daughter has had difficulty concentrating for the past week, frequently becomes dizzy, and has not been eating normally due to digestion problems. The mother also informs the nurse that their family has a history of anemia. With the family history of anemia, it appears that this is the likely diagnosis. However, in order to properly diagnose and treat the patient, not only must her symptoms and family history be considered, but also factors such as gender, ethnicity, age, and behavior. This poses the question: How do patient factors impact the incidence and prevalence of different types of anemia?

To Prepare

• Review Chapter 20 in the Huether and McCance text. Reflect on the pathophysiological mechanisms of iron deficiency anemia.
• Select one of the following types of anemia: pernicious anemia, folate deficiency anemia, sideroblastic anemia, chronic inflammation anemia, or post-hemorrhagic anemia. Identify the pathophysiological mechanisms of the anemia you selected.
• Consider the similarities and differences between iron deficiency anemia and the type of anemia you selected.
• Reflect on how patient factors such as genetics, gender, ethnicity, age, and behavior might impact these anemic disorders.

By Day 3

Post an explanation of the pathophysiological mechanisms of iron deficiency anemia and the anemia you selected. Compare these two types of anemia, as well as their potential causes. Finally, explain how genetics, gender, ethnicity, age, and behavior might impact the anemic disorders you selected.

Assignment: Application: Systems Theory

As noted in the Learning Resources, systems theory provides a meaningful and beneficial means of examining challenges in health care organizations. To do this effectively, however, it is essential to assess all system components, as some may be relatively healthy while others are problematic.

• Review the Meyer article, “Nursing Services Delivery Theory: An Open System Approach,” in this week’s Learning Resources. Focus especially on the information presented in Table 1 (p. 2831) and Figure 2 (p. 2833).
• Reflect on your organization or one with which you are familiar. Within a particular department or unit in this organization, identify a problem the staff is encountering.
• Using Table 1 in the Meyer article as a guide, analyze the department or unit, identifying inputs, throughput, output, cycles of events, and negative feedback. Consider whether the problem you have selected relates to input, throughput, output, cycles of events, and/or negative feedback.
• Think about how you could address the problem: Consider what a desired outcome would be, then formulate related goals and objectives, and translate those goals into policies and procedures.
• Research professional standards that are pertinent to your identified problem.
• Reflect on the organization’s mission statement and values. In addition, consider how addressing this problem would uphold the mission and values, while improving the organizational culture and climate.
