It may seem to you that healthcare has been a national topic of debate among political leaders for as long as you can remember.

Healthcare has been a policy item and a topic of debate not only in recent times but as far back as the administration of the second U.S. president, John Adams. In 1798, Adams signed legislation requiring that 20 cents per month of a sailor’s paycheck be set aside for covering their medical bills. This represented the first major piece of U.S. healthcare legislation, and the topic of healthcare has been woven into presidential agendas and political debate ever since.

As a healthcare professional, you may be called upon to provide expertise, guidance and/or opinions on healthcare matters as they are debated for inclusion into new policy. You may also be involved in planning new organizational policy and responses to changes in legislation. For all of these reasons you should be prepared to speak to national healthcare issues making the news.

In this Assignment, you will analyze recent presidential healthcare agendas. You also will prepare a fact sheet to communicate the importance of a healthcare issue and the impact on this issue of recent or proposed policy.

To Prepare:

  • Review the agenda priorities of the current/sitting U.S. president and the two previous presidential administrations.
  • Select an issue related to healthcare that was addressed by each of the last three U.S. presidential administrations.
  • Reflect on the focus of their respective agendas, including the allocation of financial resources for addressing the healthcare issue you selected.

Use the Agenda Comparison Grid Template found in the Learning Resources and complete the Part 1: Agenda Comparison Grid based on the current/sitting U.S. president and the two previous presidential administrations and their agendas related to the public health concern you selected. Be sure to address the following:

  • Identify and provide a brief description of the population health concern you selected and the factors that contribute to it.
  • Describe the administrative agenda focus related to the issue you selected.
  • Identify the allocations of financial and other resources that the current and two previous presidents dedicated to this issue.
  • Explain how each of the presidential administrations approached the issue.

Sarah Green has been an accountant for 12 years. She worked in a small medical practice for two years after earning her BS in Accounting and then chose to work freelance for five years. For the past five years she worked at a large firm, Stokely and Associates, where she brought in more new clients than 92% of her colleagues and received a higher satisfaction score from clients than 90% of her coworkers.

Sarah had to move to a new city to care for her sick mother. This meant finding a new accounting position. She was excited to be hired at the large accounting firm, Parker and Mendez Accounting, LLC, right away. Unfortunately, she learned after 4 months that she was being paid less than her coworkers who had equal or less experience than she did. When she asked the hiring manager about the discrepancy, she was told that the organization’s policy is to pay in a range based on previous pay. The hiring manager did admit to being surprised that her previous salary was at the low end of the expected range given her successes and performance reviews, but the policy is there for a reason and making an exception defeats the purpose of the policy.

After researching her industry and old employer, Sarah learned that her previous employer regularly paid women less than men even when the women had more experience or brought in more clients. Her current pay was impacted by this discriminatory practice.

The report to the VP of HR should include two parts:

Part One

  • A summary of Sarah’s case.
  • Address which of the EEOC acts the previous organization violated.
  • An explanation of the ethical and legal obligations of the current organization.
  • A proposed solution for Sarah.
  • A proposed change in policy for determining starting pay at the current organization.

Part Two

An outline of a training session for all hiring managers and benefits and payroll specialists. Each of these sections should be included and explained briefly but with enough detail that the decision-makers reading the report are fully informed and can approve the training.

Example: effect of smoking cessation program on smoking pregnant women. A sample of 30 women from the same hospital (or GI clinic) was assigned to an experimental or control group. Smoking status was measured by self-report and biochemical measures. Data were collected before random assignment to groups and after three follow up points: after the first week of the session, in five months and after delivery of the baby. During educational sessions, nicotine replacement gum was provided fo the experimental group while the control group received brochures about how smoking affects pregnancy and lactation.
Non-experimental research can be just as impressive, but we cannot draw the same conclusions from it as we can with experimental research. The non-experimental study is usually descriptive or correlational. That means that we are either describing a situation or phenomenon just as it is, or we are defining a correlation between two or more variables. All without any intervention from the researcher meaning that we do not manipulate any variables (e.g., change the conditions that an experimental group undergoes) or randomly assign members to a control or treatment group. Without this level of control, we cannot determine any causal effects. High chance of bias (McNiff, Petrick, 2018). While validity is still a concern in non-experimental research, the concerns are more about the validity of the measurements, rather than the validity of the effect.
Example of correlational research: a randomized trial of 100 breastfeeding mothers, we arranged to determine the relationship of maternal age and anxiety in breastfeeding women during the immediate postpartum period. The result would be: A weak but significant negative relationship exists between age and anxiety; as age increases, anxiety decreases. As we can see Correlational research measures the statistical relationship between two or more variables and the degree of the relationship (cause/effect).

Cultural and Health Belief Systems 

A worldview refers to an extensive and comprehensive outlook on life, reality and the universe. A worldview can be cultural, philosophical, liberal or even religious (Malham, 2017). It shapes an individual’s perspective on existence and trickles down to their daily activities and beliefs. It is indicative of a person’s view point, attitudes and beliefs. An individual’s worldview is also the nucleus of their cultural identity which is characterized by their beliefs, assumptions, values and attitudes, all derived from the socialization process in a specific cultural context. This is to say that a client’s cultural belief system refers to theories either based on culture or ethnicity that determine the way the client understands and structures their attitudes, health or otherwise. A cultural belief system is therefore a configuration of thoughts, notions, concepts and ideas in which their components are brought together by some form of functional reliance or interdependence (Daenikindt, de Koster, & va der Waal, 2017). Lastly, a paradigm refers to one of the components making up a worldview of cultural belief system. I could be a notion, theory or thought.

The magico-religious health belief system is based on a belief in supernatural or magical forces existing in the natural environment. Under this perspective, everything, including health and illness is in the hands of supernatural powers such as God or gods (De Angulo & Losada, 2017). Treatment can be by indigenous healers and is as a result of the influences of the supernatural forces. In the scientific/biomedical paradigm, however, no supernatural forces dominate. It focuses on solely on biological or physical components and processes excluding all others such as social and environmental influences. Under this paradigm, health and illness are under human control. Its characterized by determinism, mechanism, reductionism and objective materialism. Lastly, it stresses on medical or pharmacological approaches to treatment, approaches which essentially target abnormalities in biological processes (De Angulo & Losada, 2017). The holistic paradigm, unlike the scientific model asserts that all components/elements of a client’s life, be they social, physical, environmental, emotional, psychological and subjective have a bearing on their health. It is based on the laws of nature which require a balance of all the mentioned concepts. Lastly, every component is functionally important or has a role to play in an individual’s health. Their interactions can be nurturing or destructive (Fiandaca, Mapstone, & Federoff, 2017).

The professional healthcare system is inherently different from the folk care health system although there exist useful similarities. For one, professional care is characterized by professional or specialised education and training. Doctors, nurses and other professional healthcare providers undergo vigorous academic and practicum training for the to acquire the requisite skills as well as acquire practising licenses. Under the folk care system, although practitioners may undergo some form of training through apprenticeship, the same is not formal. Also, fork care practitioners such as lay midwives and spiritual healers are not licensed (Coreil, 2015). Secondly, professional healthcare is based on scientific and internationally recognized standards of health such as disease prevention, medical treatment patient ethics, etc. However, the folk care systems are based on cultural or religious beliefs and therefore differ between cultures, religions and other ethnic lines. This is because folk care systems across different settings embody the values, beliefs and treatment approaches of the particular cultural group in which its participants belong (Edelman, Mandle, & Kudzma, 2013). Lastly, as a result of the adherence to international standards, professional healthcare systems are organized, formal and structured unlike folk care systems that are diversified, informal and lack in structural uniformity. However, it is important to note that within a specific category of folk care say among rural Chinese practitioners. It is possible to identify a semblance of formality and uniformity. 

Allopathy or allopathic medicine refers to a health system that relies on mainstream medicine and other mainstream healthcare practices. It can also be referred to as conventional, orthodox, western or mainstream medicine (Trimble & Rajaraman, 2017). It is the contemporary form of healthcare experienced in civilised societies today, where healthcare professionals such as doctors, physicians, nurses, pharmacists, therapists and other healthcare professionals undergo professional training and are afterwards licensed to provide healthcare services including the diagnosis and treatment of diseases. In allopathic medicine, the dominant care system is the professional as opposed to the folk care systems and treatment is via medicinal treatment or medication, surgery, radiation, dialysis and other medical or scientific based treatments and therapies (Trimble &  Rajaraman, 2017).  Along with mainstream medicine in allopathy are other forms of care, commonly referred to as complementary and alternative medicine. These may include the use of herbs, massage, yoga, meditation, sleep therapy, dream work and intuition. However, in most cases, especially in the use of herbs, patients are required to consult with their mainstream medicine care providers, that is, doctors, nurses, and other physicians. Complementary and alternative medicine practitioners are known to use the term allopathic medicine as a form of distinction between their practice and that of mainstream medicine.
