Before embarking on any professional or academic activity, it is important to understand the background, knowledge, and experience you bring to it. You might ask yourself, “What do I already know? What do I need to know? And what do I want to know?” This critical self-reflection is especially important for developing clinical skills such as those for advanced practice nursing.  

The Psychiatric-Mental Health Nurse Practitioner (PMHNP) Clinical Skills List and Clinical Skills Self-Assessment Form provided in the Learning Resources can be used to celebrate your progress throughout your practicum and identify skills gaps. The list covers all necessary skills you should demonstrate during your practicum experiences.

Just as you did in PRAC 6635, for this Assignment, you assess where you are now in your clinical skill development and make plans for this practicum. Specifically, you will identify strengths and opportunities for improvement regarding the required practicum skills. In this practicum experience, when developing your goals and objectives, be sure to keep assessment and diagnostic reasoning in mind.

To Prepare

• Review the resources and clinical skills in the PMHNP Clinical Skills List document. It is recommended that you print out this document to serve as a guide throughout your practicum.  
• Review the “Developing SMART Goals” resource on how to develop goals and objectives that follow the SMART framework. 
• Review the resources on nursing competencies and nursing theory and consider how these inform your practice. 
• Download the Clinical Skills Self-Assessment Form to complete this Assignment. 

Assignment

Use the PMHNP Clinical Skills Self-Assessment Form to complete the following:   

• Rate yourself according to your confidence level performing the procedures identified on the Clinical Skills Self-Assessment Form.  
• Based on your ratings, summarize your strengths and opportunities for improvement.   
• Based on your self-assessment and theory of nursing practice, develop 3–4 measurable goals and objectives for this practicum experience. Include them on the designated area of the form.  

 

Assignment:

Determine a work area in your organization that you would like to use for assignments in this course. Focusing on one area will help you organize your work for any future assignment.  

1. Construct a presentation using PowerPoints® with voice over and create a twelve-step project charter for your workflow redesign. This should be about a health care system component (Example of a system component could be the admission process into an emergency room) and frame the project in a quality-improvement modality by establishing a plan for improvement. In your text book page 228-229 list the twelve questions that must be answered for this assignment.
2. Construct a PowerPoint® presentation of a workflow design for the same health care system component using appropriate Yourdon symbols and conventions.
3. Include these steps to create your workflow redesign:
   1. identify process to be mapped;
   2. identify and involve individuals who perform the tasks;
   3. map the current state; assess current state workflow;
   4. identify opportunities for improvement;
   5. identify data to measure redesign outcomes;
   6. map future “to be” process; test new workflows and processes;
   7. train on new workflows and processes;
   8. go live with the new workflows and processes;
   9. and analyze data and refine workflows and processes

The PowerPoints® should include a title page, the twelve-step project charter and then your workflow redesign for the same health care component, then a reference page.

  

Leadership and the Change Process

What examples of leadership have you seen in working with others at your practicum setting as well as in other situations? What have you observed that inspires you to be an effective leader? What challenges have you noticed? How does this relate to your personal leadership style? How can you, as a leader, facilitate the translation of evidence into practice? What leadership skills aid this process?

For this Discussion reflect on what makes a leader effective and how insights from theory as well as previous endeavors can be applied to planning for and implementing a change related to the translation of evidence into practice.

To prepare:

• Consider      evidence-based changes currently underway or recently achieved in your      practicum setting. Analyze the practicum environment, examine data, and      refer to theory to identify a need for a change you would propose.
• Reflect      on examples of how leadership influences the translation of evidence into      practice from within your practicum setting.
• Identify      an evidence-based change appropriate for your practicum setting.
• Think      of two or three instances that demonstrate how you have been able to—or,      if not applicable, how you could—apply leadership skills in your practicum      setting. Then, reflect on the results of your DiSC Assessment, completed      in NURS 8300. What strengths and/or challenges have you encountered, or      might you encounter based on your personal leadership style? In what ways      do you need to grow as a leader?

By tomorrow Wednesday 3/6/19 6 pm write a minimum of 550 words essay in APA format with at least 3 references (see list or required readings below). Include the level one headers as numbered below: 

Post a cohesive response that addresses the following:

1) Based on your practicum experiences, discuss the influence of leadership on the translation of evidence into practice. Provide specific insights gained from theory as well as from changes currently underway or recently achieved in your practicum setting.

2) Propose an evidence-based change for your practicum setting. Be sure to justify the need for this change using data.

3) Explain how you might apply theory as well as observations gathered from your practicum site to planning for and implementing the proposed change.

4) How are you developing your leadership skills in your practicum setting? Which aptitudes or experiences do you think are most important to your role as a leader in facilitating the translation of evidence into practice? If possible, relate this to specific learning objectives you developed during Week 1 (see attached file for learning objectives).
