Discussion 10.2: Workflow Design Team

 A group of physician offices is in the process of implementing a new clinical documentation system to replace a paper-based process. The group already uses an electronic patient registration system. You have been asked to create a multidisciplinary workflow design team to address concerns with system communication. What are some work processes that may be redesigned as a result of the implementation? How could the system improve the workflow? What team members would you assign? Explain your reasoning. 

For Week 2, you will examine how patient factors may influence pharmacokinetic and pharmacodynamic processes of pharmacotherapeutics used in the treatment of cardiovascular disorders.  You also explore ways to improve drug therapy plans for cardiovascular disorders based on patient factors and overall health needs.  

When writing your Week 2 Assignment, consider the following scenario:

Patient HM has a history of atrial fibrillation and a transient ischemic attack (TIA). The patient has been diagnosed with type 2 diabetes, hypertension, hyperlipidemia and ischemic heart disease. Drugs currently prescribed include the following:

·  Warfarin 5 mg daily MWF and 2.5 mg daily T, TH, Sat, Sun

·  Aspirin 81 mg daily

·  Metformin 1000 mg po bid

·  Glyburide 10 mg bid

·  Atenolol 100 mg po daily

·  Motrin 200 mg 1–3 tablets every 6 hours as needed for pain

Must use 3 references from 2015 or newer

Module 1 Assignment: Case Study Analysis

An understanding of cells and cell behavior is a critically important component of disease diagnosis and treatment. But some diseases can be complex in nature, with a variety of factors and circumstances impacting their emergence and severity.

Effective disease analysis often requires an understanding that goes beyond isolated cell behavior. Genes, the environments in which cell processes operate, the impact of patient characteristics, and racial and ethnic variables all can have an important impact.

An understanding of the signals and symptoms of alterations in cellular processes is a critical step in the diagnosis and treatment of many diseases. For APRNs, this understanding can also help educate patients and guide them through their treatment plans.

In this Assignment, you examine a case study and analyze the symptoms presented. You identify cell, gene, and/or process elements that may be factors in the diagnosis, and you explain the implications to patient health.

To prepare:

By Day 1 of this week, you will be assigned to a specific case study for this Case Study Assignment. Please see the “Course Announcements” section of the classroom for your assignment from your Instructor.

The Assignment (1- to 2-page case study analysis)

Develop a 1- to 2-page case study analysis in which you:

• Explain why you think the patient presented the symptoms described.
• Identify the genes that may be associated with the development of the disease.
• Explain the process of immunosuppression and the effect it has on body systems.

By Day 7 of Week 2

Submit your Case Study Analysis Assignment by Day 7 of Week 2.

CASE STUDY

Scenario 4: A 65-year-old obese African American male patient presents to his HCP with crampy left lower quadrant pain, constipation, and fevers to 101˚ F. He has had multiple episodes like this one over the past 15 years and they always responded to bowel rest and oral antibiotics. He has refused to have the recommended colonoscopy even with his history of chronic inflammatory bowel disease (diverticulitis), sedentary lifestyle, and diet lacking in fiber. His paternal grandfather died of colon cancer back in the 1950s as well. He finally underwent colonoscopy after his acute diverticulitis resolved. Colonoscopy revealed multiple polyps that were retrieved, and the pathology was positive for adenocarcinoma of the colon.

 During admission we complete our patient risk assessment and a physical plant risk assessment. our safety committee must provide ongoing staff training to ensure awareness of potential risks on units, Installation of products that reduce risk in all patient areas and routine inspections of inpatient. our psychiatric units to ensure safety levels are maintained. The care and safety of psychiatric patients at risk of harm to themselves or others, and the staff providing care are our primary concerns. A ligature risk (point) may include anything which could be used to create a sustainable attachment point such as a cord, rope, or other material for the purpose of hanging or strangulation. Also, the staff should monitor shower rails, coat hooks, pipes and radiators, window and door frames, ceiling fittings, handles, hinges and closures. Common ligature points and ligatures are doors, hooks/handles, windows frames, belts, sheets, towels and shoelaces. Staff find any ligatures risk immediately reported to the supervisor. The general members are in the group include MHTA, Nursing administer, risk management team, safety officer. Any concerns raised by workers or supervisors; the correction plan is taken immediately. The safety committee meeting will take place every two weeks. The safety committee must publish once a month report to our department heads. The nursing administrator provide information to each ward If any concerns or any issues to address and followed by in-service to each employee. They are responsible for planning, placing and frequent education sections. They should understand the issues and applicable to daily life and working environment.
