Informed consent is obtained after a client receives complete disclosure of all pertinent information provided by the provider regarding the surgery or procedure to be performed. The nurse may obtain informed consent only after the client verbalizes understanding of the potential benefits and risks associated with the surgery or procedure.

Elements of Informed Consent:

  1. The individual giving consent must fully understand the procedure that will be performed, the risks involved, expected/desired outcomes, expected complications/side effects, and alternative treatments or therapies available.
  2. Consent is given by a competent adult, legal guardian or designated power of attorney (DPOA), emancipated or married minor, parent of a minor, or a court order.
  3. A trained medical interpreter must be provided when the person giving consent is unable to communicate due to a language barrier.
  4. The nurse’s role is to witness the client’s signature and ensure the provider gave the necessary information and that the client understood and is competent to sign. If it has been identified that the client does not understand the procedure or is not competent to sign the informed consent, the nurse must STOP the process and the physician must be notified immediately.

A consultation occurs when a professional provides expert advice in a particular area and determines what treatment or services the client requires. The nursing intervention evolves around facilitating coordination with other health care providers throughout the continuum of care in order to protect the client from conflicting and potentially dangerous treatments.

A referral is a formal request for a special service by another care provider so that the client can access the care identified by the primary care provider or consultant. Some examples of referrals include physical therapy, wound care, and hospice. The nursing intervention continues to evolve around monitoring the client’s response and progress.

A mother expecting her first child miscarried at home on June 22, 2010. The pregnancy was six months along. An ambulance was called at 4:57 a.m. The EMTs helped the mother to the stretcher and then went inside to retrieve the fetus from the bathroom floor. The baby was seen moving its head. The EMTs requested ALS to the scene. The baby was placed inside a small container. The ALS personnel visually assessed the fetus and stated the fetus was “non-viable”. There was never a fetal heart check in the field. Mother and fetus were transported to the hospital arriving at 5:16 a.m.

At the hospital, a nurse noticed that the fetus was warm and had a heartbeat. The baby was raced to the special care nursery and placed on a warmer. The staff then proceeded to resuscitate the baby. The baby was dusky and noted to have a heart rate of 30 with respirations of 6-8 at 5:40 a.m., and cardiopulmonary resuscitation was initiated. The oxygen saturation was 2-10%. The baby was intubated at 5:55 a.m. At 6:05 a.m., the blood pressure was 44/24. By 6:15 a.m., the baby was on a ventilator with oxygen saturation of 96%, a heart rate of 102, but remained dusky. By 6:30 a.m., the baby’s heart rate was 120 and blood pressure 52/24.

At 7:45 a.m., the baby was transported to a Boston hospital NICU for further care and treatment. Unfortunately, the baby died on August 10, 2010 at age 1 month and 16 days, not from prematurity but rather from brain damage due to lack of oxygen.

The plaintiffs’ claimed the accepted standard of care for basic and paramedic emergency medicine technicians in Massachusetts in 2010 required basic and paramedic EMTs to provide appropriate evaluation and treatment by following established protocols. It is beyond the scope of practice for EMTs at any level to make determinations in regard to viability of a patient. As a result of the negligence, the baby was improperly designated as “non-viable”, was placed in a Pizzeria Uno’s delivery plastic bag inside of a box with a lid on it further depriving the baby of oxygen, and as a result was not resuscitated for the first several minutes of life.

Information literacy is obtained by being able to collect, utilize, manage, integrate, and produce information and data while having the information skills to do so effectively (Forster, 2015) This literacy is important to nursing research because nurses need to be able to know how to decide what information is needed in research, how to properly obtain and review this research, all while being able to teach it effectively (McGongile & Mastrian, 2018). Information literacy is an ongoing battle for nursing students to be proficient in. In 2017, Willson and Angell performed a study to develop a rubric on measuring information literacy skills of which are crucial to undergraduate nurses’ development as researchers and health professionals. In this study they were able to develop a rubric and most nursing undergraduates showed low to fair competencies in information literacy.  

Professional online databases and search engines both provide a source for an expansive amount of information. The difference between them is that professional online databases have exclusivity and generally require a subscription acquired through professional or academic associations. Search engines provide a wealth of information as well but there is no way of knowing the validity behind the information and is available to everyone.

An example of a professional online database is PubMed. Something unique about this database is that by default PubMed lists its corresponding records by date in which they were uploaded to the database.
