Care Of Adults – 2025

Hello, please are two tasks. Task # 1 is a Care Plan, below I attach two forms in words that must be completed. Please below I attach a document with an example of a Nursing Care Plan that will serve as a guide to make mine. Task # 2 is a Case Study that has two items a and b  which must be answered independently.

Required Textbooks and Reading Material:

Touhy, Theris DNP, and Jett, Kathleen, (2018). Ebersole and Hess’ Gerontological Nursing & Healthy Aging, 5th Edition. Elsevier. ISBN: 9780323401678

Task # 1

Care Plan

Medical Diagnosis: Liver spots

Age spots – also called liver spots and solar lentigines – are flat gray, brown, or black spots. They vary in size and usually appear on the face, hands, shoulders, and arms – areas most exposed to the sun. Though age spots are very common in adults older than age 40, they can affect younger people as well. True age spots are harmless and don’t need treatment, but they can look like cancerous growths. For cosmetic reasons, age spots can be lightened with skin-bleaching products or removed. However, preventing age spots – by avoiding the sun and using sunscreen – may be the easiest way to maintain your skin’s youthful appearance and to avoid these dark skin spots.

Task # 2 Case Study

Case Study: Health Care of the Older Adult

1. The nurse working at the senior center notices Mrs. Jones, a 78-year-old, crying. The nurse approaches Mrs. Jones and asks if she needs help. Mrs. Jones states “I am so embarrassed. I had another accident, and my pants are all wet. It is like I’m a baby. I never should have come to the senior center.”

a. What factors may be contributing to the urinary incontinence?

b. How should the nurse respond to Mrs. Jones?

Discussion: Health Policy In Population Health – 2025

Health Care Reform in Massachusetts

Healthcare funding and coverage in the United States have been controversial issues. Massachusetts passed the nation’s first law that required residents to carry health insurance.

· What were some of the benefits of the law, and what were some of the challenges? How could the policy have been improved?

· Does the law take into account the population health approach to health and wellness?

· Why or why not?

Nursing Role And Scope – 2025

Option 2: Consider the following scenario:

You are caring for Mr. Jones, who was admitted to the hospital 4 days ago for treatment of injuries sustained in a motor vehicle accident. His diagnoses are fractured right femur, right shoulder dislocation, and closed head injury. Mr. Jones also has chronic hypertension, which has been worsened by his failure to take his medication for several days before the accident and by the stress of the accident. Mr. Jones is 48 years old, married, and has three children aged 15, 10, and 4 years. He is the owner of a small, home-based plumbing company. Two other plumbers work for him. His wife does not work outside the home but does some of the office work for his company. The family lives in a small house in a relatively rural area, about 25 miles from the nearest city. Mrs. Jones is a quiet, shy person who does not drive very much outside of her immediate area. Mr. Jones is the dominant figure, making all major decisions in both the business and the family. The family’s financial situation could be described as “just surviving month to month.” The family belongs to a small Baptist church in their community but are not active because Mr. Jones usually considers himself “on call” every weekend. Mr. Jones is a Native American and has no family in the area. Mrs. Jones is from the area and has one sister and an elderly mother living nearby. On the basis of the above information, and the information from your text, describe what you think would be the impact of this illness on Mr. Jones and his family with respect to the questions that follow.

  1. What stage of illness do you think Mr. Jones is demonstrating? Give a rationale for your answer.
  2. On the basis of references to Mr. Jones’ cultural background and previous behavior, how do you think Mr. Jones would behave as a patient while in the hospital? Would he be cooperative? Would he tell you if he had pain? Would he refuse treatment? What internal and external influences might be factors?
  3. What internal, external, and interpersonal stressors might be operative for Mr. Jones? List as many as possible.
  4. What stressors might be factors for this family? List as many as possible.
  5. What is the potential impact of Mr. Jones’ illness on the family system?
  6. As a nurse, what could you incorporate in your plan of care to assist Mr. Jones and his family with their family disruption?

How Does Policy Influence Your Decision? – 2025

You  are the evening house supervisor of a small, private, rural hospital.  In your role as house supervisor, you are responsible for staffing the  upcoming shift and for troubleshooting any and all problems that cannot  be handled at the unit level.  Tonight, you receive a call to come to the emergency department (ED) to  handle a “patient complaint.” When you arrive, you find a Hispanic woman  in her mid-20s arguing vehemently with the ED charge nurse and  physician. When you intercede, the patient introduces herself as Teresa  Garcia and states, “There is something wrong with my father, and they  won’t help him because we only have Medicaid insurance. If we had  private insurance, you would be willing to do something.” The charge  nurse intercedes by saying, “Teresa’s father began vomiting about 2  hours ago and blacked out approximately 45 minutes ago, following a  14-hour drinking binge.” The ED physician added, “Mr. Garcia’s blood  alcohol level is 0.25 [2.5 times the level required to be declared  legally intoxicated], and my baseline physical examination would  indicate nothing other than he is drunk and needs to sleep it off.  Besides, I have seen Mr. Garcia in the ED before, and it’s always for  the same thing. He does not need further treatment.”  Teresa persists in her pleas to you that “there is something different  this time” and that she believes this hospital should evaluate her  father further. She intuitively feels that something terrible will  happen to her father if he is not cared for immediately. The ED  physician becomes even angrier after this comment and states to you, “I  am not going to waste my time and energy on someone who is just drunk,  and I refuse to order any more expensive lab tests or X-rays on this  patient. I’ve met the legal requirements for care. If you want something  else done, you will have to find someone else to order it.” With that,  he walks off and returns to the examination room, where other patients  are waiting to be seen. The ED nurse turns to look at you and is waiting  for further directions.  ASSIGNMENT:     How will you handle this situation? Would your decision be any easier if  there were no limitations in resource allocation? Are your values to  act as an agent for the patient or for the agency more strongly  developed?
