Develop a case study and a plan of care, incorporating current mobile App technology:

1. Select a population. Define your population by gender, age, ethnicity, socioeconomic status, spiritual need, and healthcare need.  Apply concepts learned in course to identify healthcare needs specific to the population and access to care (Utilize your textbook Chapters 1-25, and identified Websites).  Also use at least two references within the five years.
2. Develop a case study for a patient in your chosen population.
3. Define a provider level of care that includes telehealth, alternative therapies, and mobile App technology discussed in this class. Describe how telehealth could impact the care delivery of this patient.

Identify a quality improvement opportunity in your organization or practice. In a 1,250-1,500 word paper, describe the problem or issue and propose a quality improvement initiative based on evidence-based practice. Apply “The Road to Evidence-Based Practice” process, illustrated in Chapter 4 of your textbook, to create your proposal.

3. Discuss how the results of previous research demonstrate support for the quality improvement initiative and its projected outcomes. 2. Summarize the main idea of the research findings for a specific patient population. The research presented must include clinical findings that are current, thorough, and relevant to diabetes and nursing practice.
3. Provide a descriptive and reflective discussion of how the new tool or intervention can be integrated into nursing practice. Provide evidence to support your discussion.
