Capstone Project Milestone 1:

Practice Issue and Evidence Summary Guidelines

Purpose

Clear identification of the practice issue is the first step in evidence-based nursing. Next, the evidence is reviewed to determine the best intervention to change practice outcomes. Completion of the milestone will include identification of the practice issue using the ACE Star model of Knowledge Transformation and a review of the evidence that will support an intervention that will change outcomes. The evidence summary will be conducted through the ‘breaking down’ of a systematic review on your topic for your change project.

Course Outcomes

This assignment enables the student to meet the following course outcomes:

• CO4: Develops and outlines a scientific, systematic decision-making process to integrate critical thinking with clinical judgment to assure safe and effective outcomes. (PO #4)

• CO8: Selects evidence for best practice when planning professional nursing care for individuals, families, aggregates, and communities. (PO #8)

Due Dates

Milestone #1 consists of the completion of two worksheets (Practice Issue Worksheet) and (Evidence Summary Worksheet). Submit the completed worksheets to the Dropbox by Sunday, 11:59 p.m. MT at the end of Week 3.

Points

Milestone #1 is worth 175 points (75 points for the Practice Issue and 100 points for the Evidence Summary).

Practice Issue Worksheet Directions

1. Practice Issue Worksheet:
a. Read the guidelines including the grading rubrics below.  You will use this worksheet to document the practice issue presented and approved by your instructor in the Week 2 Discussions.
i. Your practice issue will be the same for all three Milestone assignments in this course.
b. Please type your answers directly into the worksheet.

Evidence Summary Worksheet Directions

2. Evidence Summary Worksheet
a. Read the guidelines including the grading rubrics below.  You will use this worksheet to incorporate your evidence summary into your Week 4 Milestone 2 assignment.
b. Find a systematic review article in the Chamberlain library on your topic. Your Systematic Review Article must be peer-reviewed and include a narrative summary.
i. Go to the Chamberlain Library, click on RNBSN; NR451 Capstone course. You can find systematic reviews from many databases within the library, or by searching from the library homepage main search bar.
ii. To search, combine the phrase ‘systematic review’ with your topic using the Boolean search operator AND. For example, if you are looking for a systematic review on fall prevention, you would use this Boolean search phrase: systematic review AND fall prevention.
c. Type in your practice issue in the search tab; choose a systematic review article with a narrative summary to complete your worksheet.
d. Please type your answers directly into the worksheet.

Finance And Economics In Health Care Delivery – Discussion 11 (Grading Rubic Attached And Video Attached) – The Final One – 2025

Process Flow Analysis

Have you ever encountered a process that had too many steps or was overly complicated? Perhaps you examined the process and looked for areas where you could remove steps or simplify its flow. If you did, you were conducting a process flow analysis. Process analysis is an important step in determining the financial well-being of an organization and pinpointing areas where changes could help improve the bottom line. In this Assignment, you will create a process flow analysis (also known as workflow diagramming) for an activity in your own organization (or one with which you are familiar).

To prepare:

Review the information in the Learning Resources (including the Media) for this week dealing with how to perform a process flow analysis.

Identify a process that is completed on a consistent basis in your organization (or one with which you are familiar). Examples of a process include such things as how a patient is moved through the system, or what must occur to prepare an examination room or hospital room for the next patient.

Create a simple flow analysis map detailing the process using Microsoft Word using one of the methods below:

Select SmartArt from the Insert menu and select a diagram that matches the flowchart structure you would like to use.

Select Shapes from theInsert menu and select the shape that matches the flowchart piece you would like to use. Continue inserting shapes and connect them using the arrow shapes.

Reflect on the ease or difficulty of obtaining the necessary information to create your flow analysis map.

Identify “white space” as defined in the article, “Hospital Operating System: Unleashing Throughput Potential” found in the Learning Resources for this week.

Determine strategies for improving the process flow.

Reflect on the role of the process flow analysis in financial decision making and how it can have positive impacts on the process. Attach your Flow Analysis Map to your Discussion post. (Very Important)

Describe the process you analyzed and how you went about gathering the necessary information to create your flow analysis map. Identify specific areas of white space that need to be addressed and propose strategies for improving the process. Analyze how process flow analysis can be a useful tool in financial decision making and the ramifications of not using it. Be sure and include your flow analysis map when you turn in your paper.

Reminder: This Discussion assignment serves as your Portfolio Assignment for this course. Be sure to save this Discussion Assignment on both your hard drive and on an external drive.

Describe the process you analyzed and how you went about gathering the necessary information to create your flow analysis map. Identify specific areas of white space and propose strategies for improving the process. Analyze how process flow analysis can be a useful tool in financial decision making and the ramifications of not using it. Be sure and include your flow analysis map as part of your post.

Respond to at least two of your colleagues on two different days using one or more of the following approaches:

Share an insight from having read your colleagues’ postings, synthesizing the information to provide new perspectives.

Offer and support an alternative perspective using readings from the classroom or from your own research in the Walden Library.

Make a suggestion based on additional evidence drawn from readings or after synthesizing multiple postings. (Will Send Responses Soon)

DISCUSSION BOARD

The Aging Process

The aging process is a complex interaction of genetic and environmental factors; therefore each individual ages differently. For example, changes in body composition, body weight, and body systems are common effects of aging. It is essential for those who are aging to maintain a healthy lifestyle to reduce the risk of disease and infection. However, the end result of aging inevitably is death. Appropriate preparation for death contributes to well-being of family and friends and brings peace of mind to the dying.

For this Discussion, consider the impact of aging on personal health and wellness and the importance of preparing for death.

To prepare for this Discussion:

  • Review Chapter 14 in Health: The Basics, 10th edition. Pay particular attention to aging and preparing for death.

  • Complete the Chapter 14 Personal Health Assessment “Are You Afraid of Death?” in Health: The Basics, 10th edition.
  • Think about preparing for death.

  • Review the media “Dimensions of Health Wheel.” Select two dimensions of health and consider how the aging process might affect each of them.

With these thoughts in mind:

Select one of the two Discussion Questions.

By Day 4, post a brief description of the two dimensions of health you selected. Then explain how the aging process might affect those dimensions. Finally, describe one strategy you might recommend to improve or enhance the two dimensions of health you selected for someone who is aging.

OR

By Day 4, post an example of a situation in which preparation for death was either adequately planned or inadequately planned. Explain the results and the impact of those results on family and friends. Finally, explain how this situation might influence your own preparations for death and dying.

Problem Correlates*

Relationship of Correlate to Problem

Data Supportive of Relationship +

Ex. Inadequate diet

Ex. Diets lacking in required nutrients contribute to malnutrition

Ex. All infants and children seen at the local Public Health Clinic are referred to the nutritionist because of poor diets (X.Y. personal communication, date). Of infants measured, 60% were in the lower percent for their weight/height (X report, date).

*These are factors that contribute to problem and outcomes.

+ Refer to appropriate sections of the database and relevant research of the findings in current literature.  Data in the third column could be local, state, or national statistics.

Refer to text (pg. 420-423) for additional examples.

3.    Community Health Nursing Diagnoses:

Based on your subjective and objective assessment findings above for both Weeks 1 and 2, write TWO priority community health nursing diagnoses.

You must use this format as outlined in your text page 413 (do NOT use NANDA):

Health Risk/problem of (specific injury, illness, or potential of, complication of, etc) among (specific population) related to (supporting evidence from your assessment findings).

Ø  Your CHN Diagnoses MUST be something that a community/public health nurse can do something about. See example below.

Ø  Note: Poverty is NOT a CHN diagnosis.

Ø  Remember do NOT use NANDA Nursing Diagnoses.

For example: Risk of asthma complications among children in SE Dallas County related to observations of overcrowded living conditions, lack of public transportation (may result in inability to access health care), high number of animals observed living in homes, poverty indications (may result in lack of access to medications).

Grading Rubric and Hints for this Discussion Question

Class this question  is asking you how you determine if an action is within your scope of practice.  Do not confuse scope of practice (legal regulation) and the ANA’ s standards of nursing practice (minimum competencies).

This model from the ANA web site may help you conceptualize how we assess our scope of practice.  Discussion on the ANA web site and your own Board of Nursing site and the National Council of State Boards of Nursing  will also be useful in learning how to assess your scope of practice.

Grading Rubric

Discusses several ( at least three) factors that need to be considered when determining whether or not  actions are within the scope of nursing practice?  3 points

Discusses why these factors need to be considered 3 points

Prioritizes factors that need to be considered ( which factor do you check first? second?  third? )  3 points

Cites current literature in response (course lecture is not current literature) 3 points

Describe One Or More Situations That Illustrate Social Or Cultural Humility, Or Insensitivity, Among Your Colleagues In A Manner That Fostered Improved Peer Cooperation Or Efficacy Of Patient Care Or Impeded Such Care. – 2025

Health conditions, including heart disease and respiratory infections, have no borders. What happens in one part of the world has ripple effects through the other parts, including the United States. Technology has created an even smaller world, with nurses in instant contact and eager to share information.

You and your nursing colleagues live and work around this shrinking professional world. Your nursing experience working with diverse populations presents a rich opportunity to share your international health perspectives. This exercise is a growth opportunity in terms of how we see ourselves, the world, and how we interact with our patients during the health interview and health assessment process.

In this week’s Discussion, your stories will illustrate relevant social and cultural differences among yourselves and your patients. The collective experience will expand your individual nursing knowledge; improve upon our cultural humility and perspectives, in order to foster social and culturally-sensitive health interviews and assessments. This, in turn, contributes to safer and higher quality outcomes for your patients. Consider how your sensitive care is expressed through choice of your words, maintaining a distance that is culturally appropriate, and utilizing other techniques to negotiate through established social and cultural values. This leads to patient trust and comfort and a respectful nurse-patient relationship.
