Scenario

You have recently been promoted to Health Services Manager at Three Mountains Regional Hospital, a small hospital located in a mid-size city in the Midwest. Three Mountains is a general medical and surgical facility with 400 beds. Last year there were approximately 62,000 emergency visits and 15,000 admissions. More than 6,000 outpatient and 10,000 inpatient surgeries were performed.

You have been asked by the CEO to create an intake packet for new patients that will help establish patient trust. Once you’ve created this packet, the CEO wants you to send an email to all hospital 

employees letting them know about the intake packet. You will also create a PowerPoint reviewing intake packet details for the hospital to post on its website to inform patients

Instructions

This is a great opportunity to tie everything together that you have learned! Let’s break this up into 3 parts. Deep breath – it’s easier than it seems!

Part 1: Intake Packet

• Create a New Patient Letter in a business letter format. (Here is a library resource for help writing a business letter.)
• In the letter:
  • Explain the importance of ethics
  • Explain why each part of the packet is included and how the packet is to be used 
  • Include a HIPAA/Confidentiality statement and a Privacy Pledge
  • Add a line at the bottom of the form for the patient to sign acknowledging receipt
• On a separate page, include your reference list in APA format.

Part 2: PowerPoint

• Create a 10-slide PowerPoint presentation that explains the various elements of the new patient intake packet. (Here is a library resource for help creating a PowerPoint presentation.) Make sure to use the notes section below each slide to expand on the key points.

Part 3: Email

• Write an email in a Word document that will be sent out to all hospital employees making them aware of the intake packet. (Make sure your email uses proper email formatting. (Here is a library resource for help writing a professional email.)

Week 4: Organizational Structure: Power and Lines of Authority – Collaborative Group 

Part 1: Individual

• Describe the organizational structure and the lines of authority within the healthcare system where you work. Identify the relationship between the type of power observed and the authority line.

Part 2: With Your Group

• Share an example of a power differential gap or unclear authority lines you have observed or have been involved in. What accountability issues arose from the gap? How was group communication and/or collaboration affected by this gap? What strategies would you propose leaders could use to navigate power-differential gaps and blurred lines of authority?

You have been asked for the summary to include the following components:

• Discuss the defining characteristics of telehealth
  • Include your personal definition of telehealth
  • How does telehealth impact nursing practice
• Examine how telehealth is currently integrated in healthcare
  • Describe the evidence-based practice that supports the use of telehealth in rural and community healthcare settings.
• Provide a detailed description of your recommended evidence-based strategy to implement telehealth
  • Provide rationale supporting your strategy
  • Include current evidence from the literature

What are the differences between leaders and managers? What characteristics are similar and what are different? Provide a total of three examples. First, of someone who has great managerial skills. Second, another individual with great leadership skills. Third, another person with poor managerial skills. These can be made up individuals or examples you have seen without any identifying information. Discuss how the staff of each individual would be affected by the skills of the leader/manager. Complete the Emotional Intelligence Test Discuss your scores and what they mean. What did you learn about yourself as a leader? (Note that a 10 on the EI test means strong. On the score page, click on the blue “here” under “interpreting your GEIT scores, click here” for more information on each section and what it means.  Assignment Expectations:  Length: 1500 to 2000 words total (at least 500 words per question prompt)  Structure: Include a title page and reference page in APA format. These do not count towards the minimum word count for this assignment. Your essay must include an introduction and a conclusion.

  Using SPSS and obtain an output 

1. Determine the statistics for each gender as follows: Frequency Counts, Mean, Standard Deviation, Minimum, and Maximum.
2. Graphing and Descriptive Stats in SPSS: Create a bar graph with gender (axis X) and blood sugar (axis Y).

Data Set

Use the following data set for this assignment:

1. You have a group of patients observed with a diagnosis of Diabetes and their blood sugar levels are listed below based on gender. Men: 74, 71, 75, 248, 388, 505, 42, 21.
2. Female: 62, 68, 61, 71, 68, 80, 390, 148.

Summary

Write a 250-500 word summary of your results and how this statistical analysis may be applied to your prospectus. Provide a bar graph with gender on the x-axis and blood sugar levels on the y-axis. Add your SPSS output as an Appendix to this summary.
