CLINICAL CASE DECISION INSTRUCTIONS

Grading Criteria for Clinical Case Decisions:

The highest potential points for each Clinical Case Decision Question are 15-points.  Students are required to upload their response as a word document attachment to the assignment drop box.

Each uploaded response is worth a possible 15 points.

1. Your comments should be substantiated and substantive. Postings require two citations from 1 scholarly journal and/or 1 outside textbook. Textbooks assigned to this course may be used for an additional citation only.   
2. Each of the two references must be from a different reference source.
3. To gain full credit for the assignment, the response must be a full page, but no more than three (3) pages in length, which includes the reference list.
4. Students are required to use APA format, proper citations, and references.  Students using direct quotes from referenced sources in the body of the paper must include quotations.
5. Students will be assigned to a question.

Clinical Decision Case study.

Begin with a Head-Toe assessment, you decide if it should be treated as episodic with rationale.

You have been assigned two (2) cases to provide analysis. All cases should include the following

a. Pathophysiology and pharmacology of the disease

C1.  Mr. Bush, a 45-year-old middle school teacher arrives at the emergency department by EMS ground transport after he experienced severe mid-sternal chest pain at work. On arrival to the ED,

C2. Explain how diuretics, receptor antagonists, angiotensin converting enzyme antagonists, and angiotensin receptor antagonists reduce blood pressure in individuals with primary hypertension.

Provide a very brief overview of your clinical encounter that highlights where the error/challenge/missed opportunity occurred (300 words).

Body (700 words)

Upon reviewing the module content and your previous paper, identify TWO clinical reasoning errors that best aligned to, or potentially could have occurred during your encounter that contributed to why that encounter was challenging or perhaps did not go to plan.

Describe those TWO clinical reasoning errors chosen and discuss how these may have occurred and why. How were the errors eventually remedied? how could they have been mitigated? How could it have impacted on patient outcomes if the error was not identified? In this section you may have to recall elements/assessment data from your clinical encounter to contextualise your discussion.

Conclusion (500 words)

From your exploration, consider your future nursing practice. What did you learn? what would you do differently next time? What strategies will you use? How has this enhanced your clinical reasoning skills? This forms a reasonable portion of your paper and therefore you need to demonstrate deep reflection here. Saying ‘I would read more about…..’, etc is not sufficient. We really need to see the encounter as a whole. What have you learnt from this that has better enabled you to ‘think like a registered nurse’.

In addition to the information provided in the unit outline, a rubric is provided to guide your preparation of this assessment task. Word counts for each section are to be considered as a guideline.

Watch the “Diary of Medical Mission Trip” videos dealing with the catastrophic earthquake in Haiti in 2010. Reflect on this natural disaster by answering the following questions:

1. Propose one example of a nursing intervention related to the disaster from each of the following levels: primary prevention, secondary prevention, and tertiary prevention. Provide innovative examples that have not been discussed by previous students.
2. Under which phase of the disaster do the three proposed interventions fall? Explain why you chose that phase.
