The patient is a 68-year-old man who sustained a head injury after falling in the bathroom. He arrives in the emergency department (ED) cheerful, alert, and oriented to person. On physical examination, he has a large hematoma on his left forehead. He reports a headache in the left temporal area, which he rates as an 8/10. His wife was present during the incident. Admission vital signs are: blood pressure, 170/92 mm Hg; pulse, 92 beats/min; respirations, 14 breaths/min; and temperature, 98.7° F. The physician orders a one-time dose of acetaminophen 650 mg orally for the patient’s headache, which the nurse promptly administers. The patient reports a pain level of 5/10 at 1 hour after the intervention.

The provision of holistic care is an important aspect of Swanson’s middle-range theory. Swanson was inspired by Jean Watson’s theory of human caring and had considered the use of caring-based therapeutics and answers that will mold the practice of nursing ( Andershed, et al.,2009). Swanson (1991) defined caring “ As a nurturing way of relating to a valued other person, towards whom one feels a personal sense of commitment and responsibility “ (p 2). Swanson’s theory of caring comprises knowing, being with, doing for, enabling, and maintaining belief (Swanson, 1991).

According to Swanson  (1991), knowing is focused on listening, observing, and understanding another individual without making any assumptions. Being with is about being present, and allowing expression and sharing of feelings.  Doing for pertains to the nurse’s ability to anticipate the client’s needs and perform necessary skills that will provide comfort and protection for the individual ( Swanson, 1991). Swanson(1991) emphasized that enabling will help the individual’s growth concerning healing and self-care and the nurse should take the lead in providing information and assisting the individual to understand new information received. Nurses are encouraged to continue to promote a reciprocal discussion with the client and demonstrate support and validation of feelings. Maintaining belief is the ability of the nurse to foster a positive attitude and influence the client to feel the same. The nurse should assist the client and family to feel confident and be optimistic even in difficult situations (Swanson, 1991).

The five components of Swanson’s theory of caring encompass all the facets of the nursing metaparadigm- person, health, environment, and nursing. In providing care, nurses must take into consideration that the person is the main focus of nursing. (Mintz-Binder,2019). Swanson’s theory of caring focuses on addressing the person’s needs and providing holistic care after gathering objective information and knowing the person by utilizing effective communication.  The nurse, client, and family members need to practice a clear and structured way of communicating to appropriately address the client’s holistic needs.  The environment focuses on determining both internal and external factors that may influence the client’s health status (Mintz-Biner, 2019). The nurse needs to objectively assess the client’s living situation, availability of a support system, and how to understand these factors in achieving the best client outcomes. The nursing practice involves having an invaluable nurse-client relationship and this is realized with the utilization of the five components of Swanson’s theory of caring.

I realized that I have applied all the components of Swanson’s theory of caring in my nursing practice. I worked as a bedside clinician in Labor and Delivery unit for many years and in every client admission, I always view my client as an individual. I recognized that clients who are admitted to a hospital may have different levels of anxiety and I ensure that clients are provided with undivided attention, caring attitude and making them feel that they are heard and most importantly,  letting them know that nurses are always available to attend to their needs.

Health care organizations continuously strive to increase patient satisfaction scores. In this regard, evidence-based practices are utilized to improve patient satisfaction scores. According to Ellina, et al. (2019), caring is considered as an evaluation of service, and clients could feel satisfaction when care is rendered. Nurses consistently demonstrate a caring behavior that will mainly influence how clients evaluate care and services received during the hospital stay. Ellina, et al. (2019) emphasized that research had proven that there is a relationship between caring and hospital services and patient satisfaction. Swanson’s theory of caring provides direction for nursing practice. As nurses incorporate the five components of this theory into their practice, this will be beneficial in improving patient satisfaction scores and will significantly enhance nurses’ ability to be creative, innovative, and patient-specific in managing clients (Ellina, et al., 2019)

One of the opportunities that I see in the clinical setting that needs to be improved is staffing. Of course 2020 and COVID hit us hard for the year but some facilities were already in shambles before the pandemic. Staffing is one of those things that can breakdown a facility to its inner most core. You have to hire and provide the unit with appropriate and knowledgeable staff that can handle the unit and the patient acuity. This helps with providing the best care for the patient and it gives the nursing staff time to provide the best care to the patient they are caring for. When staffing levels aren’t met, patient mortality increases. According to Rebecca Paulsen she stresses quality of care to achieve patient outcomes, which, in turn, can lead to higher reimbursement from the insurance company. Nurse-to-patient ratios are substantially important when trying to keep infection rates, potential drug reactions, and medication errors at a minimum (Paulson, 2018). Hospital reimbursement is based on performance. If units don’t adequately staff enough professionals to provide the care that patients need based on their acuity level, reimbursement may significantly be affected (Ashe, 2018). When units have higher turnover rates, including admissions, discharges, and transfers, additional nursing time isn’t recognized in shift variability when considering staffing needs and nurse-to-patient ratios(Ashe, 2018). This is way having the unit or clinical area staffed accordingly makes the unit and the level of stress, burnout, depression among staff decrease.

The paper will begin with a background on the stress experienced by students enrolled in learning programs by putting it within the context of the Covid-19 pandemic, as presented in “Stress in Students after Lockdown Due to COVID-19 Threat and the Effects of Attending Online Classes,” which found that 69.8% of students are stressed about their studies (Raj & Fatima, 2020).

The paper will use information from the article “Understanding and Promoting Stress Management Practices Among College Students Through an Integrated Health Behavior Model”, which was published in the American Journal of Health Information in 2018. The article notes that college students are susceptible to undermanaged stress and its detrimental effects, and though they could use evidence-based stress reduction techniques (SRTs) to remedy this, many do not (Bistricky et al, 2018). The article provides a framework through which institutions can better serve students that are enrolled in online classes.

One such intervention was evaluated in the article “Evaluating an Online Stress Management Intervention for College Students,” which was published in the Journal of Counseling Psychology in 2016. The study found a number of effective techniques that reduce stress in online learners and focused on the intervention focused on present control because it has been found to be associated with a range of positive outcomes, including lower levels of depression, anxiety, and stress, controlling for a range of other variables (Hintz et al, 2016).

Going globally, the paper will analyze the efficacy of using web applications for stress management during the pandemic, as seen in a sample of Iranian college students in a study by Khademian et al (2020). This application outlines the implementation of techniques and strategies to help college students who are working online due to the pandemic.

Finally, the cumulative information will guide us to the most effective tips for successful online learning, as recommended by Walden University in their article “9 Stress Management Tips for Successful Online Learning,” found at Walden.edu. 2.  A meta-analysis method was performed in this study, to synthesize different results found between several studies. The MRU virtual library was used to access all the available research through the EBSCOhost databases. In an advanced search, the following keywords were used: “Students”, “Online”, and “Classes”. A limitation was set for research published within the last 5 years from February 2016 to February 2021 with articles published in English, Chinese, German, Korean, Polish, and Portuguese. The search provided access to 102 articles of which 1 of the studies was selected based on the topic and 101 were discarded.
