Role of health care reform in shifting the focus from a disease-oriented health care system toward one of wellness and prevention, – 2025

What is the role of health care reform in shifting the focus from a disease-oriented health care system toward one of wellness and prevention, and how does nursing fit into this shift?

Trying to find a way to defray health care costs, it was decided that it is much less expensive to prevent disease than to attempt to “fix” it after an illness occurs. The cost is much less, not only in terms of money, but in terms of human suffering. People live in communities, not health care systems. Finding ways to reach people so they live a healthy life that supports wellness remains a challenge. Cultural variation and beliefs have a profound impact.  You will have to reach deeply into your past course of Health Promotion to answer this question. How do we need to live in order to stay well? This is a very important question in relation to the current changes in relation to healthcare. Please think beyond diet, exercise and taking meds. They are important too, but true health promotion goes far beyond that limited focus.

Advocating for New Practice Approaches Through Policy

“I think one’s feelings waste themselves in words; they ought all to be distilled into actions which bring results.”
—Florence Nightingale

Through the policy process, nurses can put their feelings—as well as their experiences and expertise—into actions that create results. In this course, you have been honing your skills for critically assessing evidence with the purpose of improving practice and developing new practice approaches. As a DNP-prepared nurse, you can influence health care outcomes at the population or aggregate level. Leading policy development to address health care concerns is a vital way to achieve this.

For the past several weeks, you have been focusing on a health care issue of interest to you for your EBP Project. For this Discussion, consider how you could lead policy development in support of a proposed evidence-based practice approach that addresses that issue and is designed to promote quality.

To prepare:

  • Select one of the new practices approaches you      developed for the Week 6 Discussion (See attached file)
  • Review the Ridenour and Trautman (2009) and      Stokowski et al. (2010) articles listed in the Learning Resources. Assess      the strategies for engaging in the policy process.
  • Consider the barriers to using evidence to      affect changes in health care policy discussed in the course text.
  • Brainstorm ideas for leading policy development      related to your suggested practice approach, as well as ideas for      addressing those barriers.

literature question – 2025

5th Entry – Creative Nonfiction: In the preface to nonfiction (p. 2803 of your text), the editors tell us that creative nonfiction illustrates that “no direct duplication of reality is possible in language, that all writing is affected by the author’s point of view” (Baym et al, 2014, p. 2803).  Do you think that is true?  Can there such a thing as a truly reliable narrator?  Consider the excerpt from Danticat’s Brother, I’m Dying that you read this week as you prepare a response to this question.  How is this point made (or not) in Danticat’s writing?

paper help – 2025

Answer only two of the inquiries below.

2. A number of medical centers around the United States now offer “finders’ fees” to physicians for referring patients to researchers who are conducting trials of new drug therapies, the side effects of which are not yet known.
one researcher, for example, was offering physicians a $350 payment for each referred patient who enrolled in the research project.   Many physicians accept the fees and make the referrals, apparently without suffering pangs of conscience.  Are their actions ethical?

3. From 1940 to 1970, more than 4,000 radiation experiments were performed on tens of thousands of Americans, many of them poor and uneducated, without their informed consent. Examples of alleged incidents: children in a Massachusetts orphanage were fed radioisotopes; 829 pregnant Tennessee women were fed radioactive iron; patients in Rochester, New York were injected with plutonium; cancer patients in Cincinnati received heavy doses of gamma rays.  Not all of these experiments can be attributed to researchers’ ignorance of the harmful effects of radiation; the main purpose of the experiments was to identify those effects rather than to cure the patients. Even so, the researchers do not seem to have thought they were committing a moral offense. Were they?

10. The consciences of the people in the following cases are confused.  As a result, the people cannot decide whether the actions they are contemplating are morally right.  Decide for them and present the rationale for your position.

d. An airline pilot goes for his regular medical checkup.  The doctor discovers that he has developed a heart murmur.  The pilot has only a month to go before he is eligible for retirement.  The doctor knows this and wonders whether, under these unusual circumstances, she is justified in withholding the information about the pilot’s condition.
